The Department of Neurology at the University of Vermont Medical Center is seeking a Social Worker to support outpatient clinical practice. The Social Worker will provide supportive counseling, conduct psycho-social assessments and provide referrals to holistically support patient care. Critically, the Social Worker will assist patients and caregivers in their care coordination within and outside the medical center over the arc of their illnesses. Working closely with the multi-disciplinary team of MDs, NPs, RNs, and others, the Social Worker will assess psychological and cognitive functioning and organize and lead support groups for patients and caregivers impacted by neurological conditions.

Responsibilities

• Conduct comprehensive clinical psychosocial assessments.
• Develop and implement treatment plans and maintain plan updates in electronic medical record.
• Advocate for and obtain appropriate services.
• Provide or obtain concrete and supportive services.
• Participate in supervision, staff meetings, in-services and trainings.
• Other duties as assigned.

Minimum Qualifications

• Requires a master degree or equivalent in education and experience, plus three years of related experience.
• Must hold a Master's degree (minimum) in Social Work Applicant must hold current State of Vermont, Social Work License. A valid LICSW license is required to apply for this position. Training or certification may be required in a specific area: Case Management, Group Therapy, Cognitive Therapy, Dialectical Behavioral Therapy, Eye Movement Desensitization and Reprocessing, or Substance Use Treatment.

Preferred Qualifications

• Ideal candidate will have demonstrated knowledge and experience with neurology patients and skill in oral communication, written communication, problem solving/assessment, teamwork, team building/team leader, group process facilitation, documentation accuracy, customer service, interpersonal relations, creativity/innovation, adaptability, decision making, and conflict resolution.
• 3-5 years of social work experience with inpatient or outpatient hospital experience working with neurological diagnoses preferred.
• Experience with Headache, Multiple Sclerosis, and Neuromuscular Diagnoses a plus.
• Case Management accreditation by a nationally recognized accrediting body for case management preferred. (Examples: CCM, ACMA or ANCC certifications)
