POSITION SUMMARY

The Pediatric Behavioral Health Care Manager is responsible for the development and execution of patient care plans/treatment plans and care coordination activities for developmental, behavioral, and mental health care issues for patients ages newborn – 21 years and their families.

ESSENTIAL JOB FUNCTIONS AND ACCOUNTABILITIES

  • Provides direct clinical services including diagnostic assessment, development of treatment plans, counseling, and psychotherapy to children, adolescents, and their respective families/guardians.
  • Conducts and Initiatives screening and assessment of behavioral, emotional, academic, socio-economic, self-management, risk, and developmental issues.
  • Support the mental and physical health care of patients within the organization.
  • Coordinate care with the patient’s medical provider(s) and, when appropriate, other mental health providers.
  • Obtains essential demographic information, facilitates patient engagement and follow-up.
  • Establishes and maintains comprehensive, high-quality, meaningful care plans, based on documented patient needs.
  • Provides crisis intervention for patients and when relevant, their families.
  • Act as an advocate for the patient
  • Systemically track treatment response and monitor patients for compliance, changes in symptoms, or other complications
  • Support medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Initiates and coordinates education regarding treatment for patients and when relevant, their families.
  • Facilitate referrals for necessary clinical services.
  • Develop and complete relapse prevention self-management plan with patients who have achieved their treatments goals.
  • Conducts proactive outreach, phone calls, and provides appropriate follow-up and documentation according to established policy and procedure and within deadline requirements.
  • Maintains ongoing communication with the primary care physicians and other providers involved in the patient’s plan of care, updating the care plan/treatment plan as necessary.
  • Maintains a professional, collaborative, and productive relationship with providers and the care team.
  • Collaborates with team and participate in weekly review of patients’ cases for better outcomes.
  • Participates in quality improvement projects as needed and implements quality improvement initiatives.
  • Effectively utilizes all tools available to support care management. Maintains a high level of proficiency with care management tools within EMRs and other applications intended to support patient care.
  • Other duties as assigned.

REQUIRED QUALIFICATIONS

  • Education: Bachelors Degree in Social Work or Counseling, Master's preferred
  • License and/or Certification: LSW or LPC Required. An independently Licensed Social Worker (LISW) or Licensed Professional Clinical Counselor preferred.
  • Experience/Skills:
    • Strong organizational skills, ability to work independently, possess excellent time management.
    • Previous experience working with pediatric and adolescence papulation.
    • Ability to coordinate care with patient and patients’ families.
    • Ability to interact professionally with health-care practitioners, clinicians, and the public.
    • Excellent written and oral communication and interpersonal skills.
    • Technical skills- ability to effectively utilized EHR system to document, and appropriately collect data to track provided interventions for effectiveness.
    • Experience in a mental health environment
