The Licensed Clinical Social Worker will work as part of the Texas Center for Pediatric and Congenital Heart Disease (TCPCHD) at the University of Texas Dell Medical School and Dell Children's Medical Center.

The first of its kind in the nation, the Department of Health Social Work is a collaboration of Dell Medical School and the Steve Hicks School of Social Work at The University of Texas at Austin. Jointly operated by both schools, the department creates an academic home for social workers and other integrated behavioral health providers to improve the health of individuals, families and communities, create value in health care, innovate new models of health social work and integrated behavioral health practice, and build civic and community engagement and connections. The department creates a formalized structure of collaboration to support their shared interprofessional missions.

Responsibilities

  • Conducts a specialized psychosocial assessment on each patient referred for cardiac transplant. This assessment is conducted to assist the transplant team in determining a patient's psychological and social readiness for the demands and stresses associated with transplant surgery, recovery, and rehabilitation. Completes documentation in the clinical record related to interaction with individuals-served, families, and other service providers in an accurate and timely manner, and in accordance with professional ethics and with both Federal and State regulations.

  • Uses clinical expertise to provide therapeutic recommendations and interventions to patients/families through the collaborative development of a treatment plan, and provides consultation, orientation, and training to health care staff on the effects of biopsychosocial, cultural, and economic issues on the management of patient/client health needs

  • Participate in value-based research initiatives and research for patient care operational improvements. Participate in screening, tracking, and reporting of patient outcomes and social work interventions. Participate in social work/departmental meetings, continuing education opportunities, academic and research opportunities to optimize patient best practice and interdepartmental collaboration. Provide discipline specific knowledge to interdisciplinary team around mental health, cultural competency, social determinants of health, and innovative ways to approach chronic illness in patients

  • Engage in care coordination efforts and complex discharge planning with the inter-disciplinary team to ensure cohesive service delivery. Aid with locating and accessing concrete services such as housing, transportation, and immigration issues to help mitigate any barriers to care and improve quality of life. Provide information and referrals for disability services, medical insurance coverage, and other financial needs.

  • Participates in transplant team care conference and selection committee meetings. Active participation and professional development in transplant-related and social work professional organizations (i.e. OPO, NASW, STSW). Participates in the development of program policies, procedures, and processes in relation to social worker activities and transplantation. Participates in CMS and UNOS survey readiness activities.Participates in transplant quality assurance and performance improvement activities

Required Qualifications

Hold a Master of Social Work (MSW) degree from an institutional program certified by the Council on Social Work Education (CSWE). Be licensed as a Licensed Clinical Social Worker in the state of Texas. Maintain a positive, compassionate, collaborative approach when engaging with patients and their family members. Demonstrate the ability to communicate and collaborate effectively with a variety of other health care professionals (i.e., physicians, psychologists, social workers, nurses, physician assistants, physical therapists, dietitians, pharmacists, medical assistants). Maintain a highly collaborative approach to a diverse and inclusive cohort of clinical team members, as well as the local, regional and national professional communities. Possess excellent interpersonal skills; written and oral communication skills; organizational, planning, follow-up/follow-through, and time management skills. Possess strong analytical and problem-solving skills. Ability to work in a fast-paced, multi-provider practice environment. Demonstrated ability to be flexible and adaptable to change. Experience conducting assessments and evidence-based interventions.

Preferred Qualifications

Certified Clinical Transplant Social Worker (CCTSW) credential. Transplant experience. Bilingual in English and Spanish. Experience working with children, adolescents and their families strongly preferred.

$70,000 depending on qualifications
