Position Summary:

A patient navigator is a lay professional who provides resources to patients, families, and caregivers to expedite timely access to quality health care throughout all phases of the oncology care continuum. The patient navigator coordinates patient care utilizing guidelines, algorithms, and directions from the nurse navigator. The goal of the patient navigator is to work autonomously and collaboratively with the nurse navigator and the multidisciplinary care team to assist in preventing fragmented or delayed care. The patient navigator contributes to the patient navigator program development, implementation, and evaluation. The patient navigator utilizes data to collaboratively improve the navigation process and participate in quality improvement. The patient navigator identifies, analyzes, and uses new knowledge to mitigate barriers to care. The patient navigator acts as the primary contact for patients and referring providers for intake of referrals, medical record procurement, and medical history abstraction. The patient navigator makes appropriate appointments and supportive care referrals and provides information to the patient based on algorithms, patient presentation, identification of patient needs, and the direction of the nurse navigator or other multidisciplinary team members.

Associate degree preferred and/or previous experience in a healthcare setting
