Position Summary | The Office forCommunity Health at the University of New Mexico’s Health Sciences Center iscurrently hiring two Social Workers to join a team of peer healthcareworkers, including Community Health Workers, and provide rapid resourcereferrals and community connector/navigator services to high-need guests at theGateway Receiving Area located on 5400 Gibson Blvd SE, a grant-fundedcollaboration between the City of Albuquerque and UNM HSC’s Office of CommunityHealth. The Gateway Receiving Area site will operate by harm reduction principles andpractices. Staff are expected to comply with public health evidence-based andbest practices at all times while at work. You will help plan, deliver, supervise,and staff the 24/7 services at the short-term Community Connector servicessite. We are seeking a compassionate, collaborative individual withstrong organizational, supervisory and communication skills with the ability toproblem-solve efficiently, independently and as part of a team as is requiredand appropriate. Because the program is new and innovative, youwill help us create the model, applying your social work perspective, skillsand experience. Summary Under general supervision, worksclosely with managers, public health workers, other community health workers,professional students, and medical/behavioral/social services agencies toprovide short term care coordination and connection to resources and support toprogram clients to improve their health and general well-being througheducation and provision of coordination of care and resources that support programclients to improve their health and general well-being while engaging withat-risk populations with complex psycho-social histories. Providesa range of assessment and case referral services for individuals, couples,and/or families, as appropriate to the specific expectations of the position. Manage andsupervise the daily activities of a team of Community Health Workers. Mayprovide practical training and guidance to students and/or paraprofessionalstaff, as required. Minimum Qualifications | Master's degree in Counseling, Social Work, or directly related field; at least 6 months of directly related experience which may include internship and/or practicum.Certification/LicensureNM licensure as LPCC, LPC, LMHC, LMSW, LCSW, or equivalent level, OR licensed out of state and eligible for NM licensure. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. |
