Job SummaryThe Institute for Biomedical Informatics ( IBI ) has contracted with the Cabinet for Health and Family Services, Department for Medicaid Services ( DMS ) to create a team of licensed Registered Nurses that will perform assessments utilizing the Home and Community Based ( HCB ) waiver assessment tool for the frail and elderly population and physically disabled. This position will identify, develop and present information educating professional staff, families and individuals seeking HCB Services while also making sound independent determinations as to the level of care and services needed for this vulnerable population.
The majority of in-home assessments will be based in Breckinridge County, but you may be asked to complete in-home assessments in surrounding counties. You will receive mileage reimbursement and will be given flexibility when scheduling in-home assessments to meet both your needs and client needs.

Skills / Knowledge / AbilitiesKnowledge of physical and psychological development. Excellent analytical and decision making skills. Ability to communicate effectively and interact with physically disabled and elderly individuals. Ability to travel throughout the state on a regular basis with the ability to travel out-of-state for overnight trips upon occasion.
Does this position have supervisory responsibilities? No
Preferred Education/ExperiencePrefer experience working with the elderly and physically disabled. Driver’s license is highly desired.
