Description:

Position Summary
The Extra Help Work Program Participant functions as a healthcare advocate performing various patient care activities including social service coordination in clinics and communities for predominantly Spanish-speaking populations. The Work Program Participant is responsible for providing outreach, education, and social support in community settings during the week as well as on some evenings and weekends, as outlined in specific programs.
Duties & Responsibilities

  • Assist Spanish and English-speaking patients at Mile Square Health Center/UI Health hospital and clinics with completing a cancer risk assessment tool to patients during their primary care clinic visits to assess their risk of developing hereditary cancers (i.e. breast, colorectal, prostate cancer); inform provider of assessment results. Patient contact may sometimes occur during weeknights and weekends.
  • Serve as a resource person for English and Spanish-speaking patients/participants in the program; develop referral relationships with healthcare providers and other community-based service providers.
  • Schedule patient appointments via the practice management system or other hospital system; refer patients/participants to support services, including assisting patients/participants in utilizing referral services and following-up with patients/participants on the referral(s).
  • Host, participate, coordinate, and sponsor community functions, in English and Spanish, to help raise awareness of breast cancer, cervical cancer, and other cancer-related services available through the University of Illinois Cancer Center, Mile Square Health Center, and UI Health programs at large. Events may sometimes occur during weeknights and weekends.
  • Assist in the development of resource materials, preparation, and maintenance of records.
  • Provide Spanish- and English-based educational information and outreach regarding programs, healthy lifestyles, healthcare needs, associated requirements, and other opportunities to community residents and the public.
  • Participate in essential trainings regarding cancer genetics and survivorship navigation and other program duties.
  • Perform other related duties and participate in special projects and assignments as assigned.

Qualifications:

Minimum Qualifications

  • High school diploma or equivalent.
  • Possession of qualifications established for the specific program for which the candidate is applying.
  • Eligibility for employment under the provisions of the Statute and Rules governing the State Universities Civil Service System of Illinois.
