Job Summary
The University of California San Francisco's (UCSF) ACT is hiring a Nurse Practitioner to join the Division of Hospital Medicine (DHM) at Zuckerberg San Francisco General (ZSFG) as an integral member of the ACT. The ACT is a service for hospitalized ZSFG patients with unhealthy alcohol, drug, and tobacco use. ACT provides compassionate, evidence-based addiction care focusing on linkage to treatment and stigma-free, patient centered care. There may also be opportunities to provide follow-up care with our outpatient addiction medicine clinics to be determined based on clinical needs.
The NP works with patients during and after hospitalization to support them as they move towards healthy behaviors. Responsibilities include screening and assessing for substance use disorders, providing brief interventions, counseling via motivational interviewing, education about harm reduction, medication and psycho-social treatment options, and working with the ACT to link patients to primary care and addiction treatment. In addition to working with ACT members, the NP works closely with hospital staff, nurses, trainees, and physicians.
Serves as a hospital and community clinic resource and liaison for patients and their families. Assists with navigating all aspects of the healthcare system. Uses knowledge of the unit or clinic to resolve issues and identify ways to increase patient satisfaction. Collaborates with department managers to meet patient expectations, achieve quality outcomes and build a patient focused culture.

Department Description
The ACT is a consult service that works with hospital based medical teams to initiate evidenced-based addiction treatment including medication, harm reduction, motivational interviewing, and psycho-social treatment. ACT is comprised of an attending physician, three patient navigators, three licensed vocational nurses, and rotating trainees including fellows, residents, and students. Some of the ZSFG addiction clinics we partner with include the Family Health Center Bridge Clinic, Outpatient Buprenorphine Induction Clinic, and the Opiate Treatment Outpatient Program.The Division of Hospital Medicine (DHM) is the largest division in the Department of Medicine (DOM) in terms of faculty size and is a national leader in clinical care, education, research, and quality improvement. Clinically, the division runs 10 clinical services and provides care for nearly half of the patients in the adult hospital on any given day. The Division of Hospital Medicine is the national leader in the field of Hospital Medicine, and supports a diverse research program spanning a broad range of clinical questions and methodologies.
The population at ZSFG includes low-income individuals including those experiencing homelessness who reside in San Francisco. Patients include a large proportion of underserved African American, Latinx, gay, lesbian, bisexual, and transgender individuals, and pregnant and post-partum women.
