UnityPoint Health is Hiring a Pharmacy Resident Near Rapids, IA

Overview

Description of Position:

Provide a “snapshot” or the principal purpose or focus of the position, consisting of no more than three to five sentences. This summary should provide enough information to differentiate the major function and activities of the position from those of other positions.

The Pharmacy Resident is a pharmacist who is undergoing additional post-graduate training by participating in a number of intensive learning experiences designed to build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training.


Responsibilities

Essential Functions/Responsibilities:

The resident’s primary function is to develop their skills as a clinical pharmacist through participation in the pharmacy residency program as outlined in the Pharmacy Residency Program Manual. In doing so the resident will perform the following functions and responsibilities:

Operational Functions

· Oversees technical and support staff in the absence of administrative personnel

· Verifies the accuracy of medication stock in automated dispensing cabinets

· Reviews the work of other pharmacy personnel to ensure the appropriate dispensing of medications per the prescriber’s order in accordance with laws and regulations.

· Serves as a drug information resource for the health care team, patients, and community members

· Inspects pharmaceutical supplies, storage, and utilization on assigned patient care areas

· Maintains a neat, clean, orderly, and secure department and work area

Clinical Functions

· Practices pharmaceutical care activities. Reviews medication orders to assess the appropriateness of drug selection for the patient. Based on patient specific parameters (e.g. renal and hepatic function, co-morbidities, patient-specific medication compliance issues, etc), evaluates medication regimens for appropriate dosing, schedule, route, contraindications, and potential drug interactions. Contacts the prescriber as necessary for clarification or to recommend appropriate therapy or dosage changes. In accordance with established policies, procedures and protocols, specific activities may include, but are not limited to:

  • Dosing of medications using the principles of pharmacokinetics
  • Management of parenteral nutrition
  • Review of chemotherapy regimens
  • Assessment of select IV medications to determine if oral administration is appropriate
  • Provide drug and disease education in areas including diabetes management, anticoagulation, and self-administration of injections
  • Actively supports and participates in medication reconciliation
  • Evaluation and interpretation of culture and susceptibility reports for antimicrobial recommendations
  • Participation as a member of the health care team in patient care rounds
  • Evaluation of complex questions regarding medication regimens and adverse drug reactions
  • Assessment of medication treatment guidelines, protocols, medication formularies and dosing guidelines, identifying areas for improvement in these areas
  • Maintenance of the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selections

· Supports the expansion of patient care services

· Documents pharmacist interventions in the electronic health record

· Accountable for quality improvement measures and patient safety initiatives, particularly as they relate to medications and medication use evaluation (MUE) activities

· Supports the ongoing development of pharmacy staff to increase knowledge and skills

· Participates in appropriate committees, including Pharmacy and Therapeutics Committee, Pharmacy and Therapeutics Subcommittees, quality improvement committees, and other work groups

· Demonstrates a commitment to the improvement of patient care, customer satisfaction, and team member performance

· Participates in the experiential education of pharmacy students, and students of other health care professions

OTHER FUNCTIONS: The following are performed on an as needed basis:

· This position may be assigned a specific mission to address in an emergency situation to assist in disaster response and recovery as part of the Hospital Emergency Incident Command System. Situations of a critical nature may require an individual to perform multiple tasks until additional support can be obtained.

· Functions other than described above may be needed occasionally required to provide optimal patient care

· Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Qualifications

Minimum Requirements

Education:

· B.S. Pharmacy with equivalent clinical experience

Doctor of Pharmacy (Pharm.D.) preferred

Experience:

· Hospital pharmacy experience during internship preferred

· Must be able to proficiently read, write and speak English.

· Ability to work with computer systems required

License(s)/Certification(s):

· Valid driver’s license when driving any vehicle for work-related reasons. Iowa Pharmacist license in good standing or eligible for licensure

· Iowa Pharmacist license in good standing or eligible for licensure

· Must attain a license to practice Pharmacy in Iowa by August 1 of the residency program year. Must be directly supervised by a licensed pharmacist until Iowa Pharmacist license is obtained

· Must maintain current license to practice pharmacy in Iowa and be in good standing with the Iowa Board of Pharmacy Examiners to continue employment
