Under the direct supervision of a Registered Nurse, provides basic nursing care for the personal and comfort needs of the patient.
ResponsibilitiesTECHNICAL/ADMISTRATIVE/CLINICAL: Demonstrates competence in the skills (processes, procedures and equipment) necessary to carry out assigned duties as identified below. Demonstrated competence in recognizing and responding to patients physical, mental, emotional and developmental needs should be evaluated in relation to the patient population served as appropriate for each responsibility. Contributes toward accomplishment of patient’s Plan of Care goals by providing care activities as assigned by the nurse. Provides for the personal grooming, hygienic and comfort needs of the patient in a thorough manner as requested or as indicated by observation. Orients patient and family members to environment, unit/department and hospital facilities/routine. Assists the nurse in obtaining basic physical assessment data, i.e. B/P, TPR, weight, I&O.Assists with basic unit procedures, i.e. admission, discharge, transfer. Promotes, monitors and maintains a safe clean environment. Assists with identifying safety needs of patients and modifying the environment to meet these needs, i.e. accessible call light, furniture/personal item placement, etc. Performs light housekeeping functions to maintain clean, neat and orderly work/patient care areas. Adheres to established Universal/Standard Precautions. Contributes effectively to the communication needs of the unit/department. Acknowledges patients, healthcare providers and visitors upon arrival to the unit/department. Offers assistance where appropriate. Documents objective data related to performed patient care activities. Engages in collaborative communication, i.e. shift report, unit meetings, etc. Identifies and reports patient care concerns in a timely manner. Demonstrates an open honest relationship with co-workers. Practices good telephone etiquette. Performs clerical duties as assigned (i.e. basic order entry, charge entry, form filing, etc) Provides for efficient operation of unit/department by taking initiative and assuming other responsibilities as needed, i.e. obtaining equipment, restocking supplies, running errands, etc. Demonstrates competence in the implementation/administration of age appropriate care in accordance with established Practice Guidelines, Policies/Procedures and State/Federal Law. Hygiene Measures: Bath Hair Care Nail Care cleaning (not cutting) Oral Care Skin Care Nutritional Measures: Feeding (Oral) Elimination Measures: Catheter Care Peri Care Colostomy care (bag change, cleansing around stoma, release of air in bag) Activity Measures: Patient Ambulation ROM Activities Non Sterile/Non Invasive procedures: Application of Heat and Cold Application of Sequential Hose Oral Suctioning Oxygen Setup Simple Dressings Traction set-up Blood glucose testing Measuring and recording vital signs including blood pressure, pulse, respirations, temperature and oxygen saturation (pulse oximetry) Non-sterile Specimen Collection (urine, sputum, stool) Education: Meets educational/competency requirements per policy Progression toward goals from previous evaluation/orientation completion.
QualificationsEDUCATION, TRAINING & EXPERIENCE: Successful completion and documentation of one of the following: Active Certified Nursing Assistant as listed on the Iowa or Illinois Healthcare worker registry; and/or active certification in another state and awaiting certification in state of employment. Certified Nursing Assistant listed as inactive on a Healthcare worker registry who is currently employed as a Certified Nursing Assistant in a hospital or similar non-skilled/long term care facility. Previous certification and registry status must have been obtained; current employment as a Certified Nursing Assistant must be verified. Graduation from an accredited nursing program and waiting for NCLEX reexamination. Current employment as a PCT working within Trinity Medical Center that were grandfathered into the PCT position title; continued employment as a PCT must be maintained otherwise recertification will be required. KNOWLEDGE, ABILITIES & SKILLS: Must be able to read, write and follow written and oral instruction. Must have interpersonal skills to deal effectively with patients and staff. Must be able to set priorities and react rationally to situations in patient care settings. LICENSES, REGISTRATION & CERTIFICATION: Current CPR Certification required.
