The Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program (THP) provides direct support and intensive case management services to assist Transitional Aged Youth (TAY) ages 16 to 21 years, with meeting individualized goals while connecting them with resources in the community. Average case load is 12 youth. The position is collaborative and coordinates care on an interdisciplinary team.
As with all UC staff, this position is responsible for adhering to the performance standards set forth in the UC Staff Employee and UC policies and procedures.
