This role will coordinate all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling of HbMC patients in coordination with clinicians and the interdisciplinary team to optimize their clinical outcomes and improve quality of life. 

Primary Responsibilities: 

  • Counsel home-based patients their families / caregivers on basic rules of good nutrition, healthy eating habits, and nutrition monitoring to improve their quality of life
  • Assess nutritional needs, diet restrictions and current care plans to develop and implement dietary-care plans and provide nutritional counseling
  • Consult with PCPs, specialists, and HbMC clinicians / interdisciplinary team to determine nutritional needs and diet restrictions of patient
  • Advise patients and their families on nutritional principles, dietary plans and diet modifications, and food selection and preparation
  • Develop policies for food service or nutritional programs to assist in health promotion and disease control for HbMC patients
  • Develop curriculum and prepare manuals, visual aids, course outlines, and other materials used in teaching
  • Prepare and administer budgets for food, equipment and supplies
  • Plan and conduct training programs in dietetics, nutrition, for the patients, family, caregivers, and HbMC’s interdisciplinary team
  • Coordinate diet counseling services
  • Develop policies for nutritional programs to assist in health promotion and disease control
  • Incorporates national best practices, models and nutrition trends into care plans
  • Proactive disease management support for high-risk chronic diseases such as diabetes 
  • Access to reliable transportation required; if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy

Required Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Registered dietitian (RD) by the Commission on Dietetic Registration (CDR) 
  • Certified Nutritionist (CN), Certified Nutritional Consultant (CNC), Certified Clinical Nutritionist (CCN), Certified Nutritional Specialist (CNS) 
  • Graduation from an accredited four-year college or university with major course work in a field related to Food and Nutrition 
  • Licensure / certification in the state of practice, where required 
  • 1 years of nutritionist work experience 
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Preferred Qualifications:

  • Experience in providing interdisciplinary team presentations 
  • Knowledge and proficiency of basic Word, Outlook and Excel skills 
  • Exceptional interpersonal skills. Superb written and oral communication skills, and comfort level with technology. 
  • Demonstrated leadership, mentorship and teaching capability
