Clinical Services positions in Prospero are part of a community-based interdisciplinary care team of home-based medical care providers to support patients facing serious illness, including their families and care givers. What makes your clinical career greater with Prospero? The Licensed Clinical Social Worker is a member of a physician-led interdisciplinary team whose primary function is to provide assessments of the psychosocial and economic needs of the patient, provide support for patient coping with advanced illness, facilitate conversations related to the patient goals of care and end of life issues, provide caregiver support, and connect patients / families with community resources. 

Primary Responsibilities:

  • Visit patients facing serious illness in their homes to conduct a psychosocial assessment of emotional, social, and environmental strengths and challenges related to their diagnosis, illness, treatment, and home environment; Extend care and support with the use of virtual/telehealth tools when in person care is not indicated
  • Help patients and families to articulate goals of care, utilizing appropriate clinical social work treatments and intervention, while maintaining the dignity of the patient
  • Provide supportive counseling and crisis intervention to patients and/or caregivers 
  • Partner with physicians, nurse practitioners, and interdisciplinary team members to coordinate patient care plans by proactively identifying and appropriately addressing social and emotional factors related to the patient’s care
  • Maintain working knowledge of and liaison with community agencies and resources; assessing and appropriately referring patients / families
  • Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and in accordance with policies and compliance

Required Qualifications:

  • Master's Degree - level clinician in Social Work (MSW)
  • Licensed Clinical Social Worker in good standing
  • 2 years of Social Work experience, post Master's degree
  • A current and unencumbered license or certification to practice as a social worker in the state assigned
  • Ability to work within a team-oriented, dynamic environment to achieve goals and meet deadlines 
  • Ability to assimilate, analyze, draw conclusions, and make recommendations from complex data 
  • Ability to work and provide high quality patient care in a virtual/telehealth environment
  • Proven highest level of ethics and integrity 
  • Demonstrated excellent written and verbal communication, listening, and social skills
  • Comfortable with technology; willing and able to learn new software tools 
  • Ability to travel between patient’s homes in assigned territory, daily. Limited overnight stays
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Preferred Qualifications:

  • Palliative care or hospice experience
  • Experience working with patient and families dealing with serious illness and death
  • Experience working with community resources
  • Bilingual or multi-lingual
