The Care Specialist will be responsible for providing direct guidance and support to members served within Family Engagement Center(FEC) which include families, adults and youth facing complex medical and behavioral health conditions. The role also supports the nationwide team of advisors and clinicians within the Family Engagement Center on topics related to the community resources and/or spiritual support needed by adults, families, children, and youth with complex care issues. This position is responsible for providing ongoing coaching, training and support to the FEC related to the diverse needs of the families, caregivers identified in the realms of private insurance coverage (medical, behavioral, pharmacy and DME) as well as integration of community and state resources (Medicaid Waiver, education, transitions, accommodations and inclusion); natural supports, HIPAA, Civil Rights Laws, Ferpa and Title IX, Transition to Independence and/or spiritual support.

This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 7:00am – 6:00pm CST. 

*All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.

Primary Responsibilities:

  • Provide guidance and education to individual members and families via phone and email correspondence for the following topics (not all inclusive):
    • Disability issues related to private insurance
    • Characteristics of disability and relationship to ADL, current and future impact on family, social success and referrals to partnering programs
    • Community and state resources, Special Education, accommodations and inclusion, natural supports and Transition to Independence
    • Emotional Support
    • Spiritual and Emotional Care
    • Social Determinants of Health related areas
    • Stepping in to offer support in cases of member or staff emergencies
  • Provide guidance and training to advisors via the phone, in-person and instant message for the following topics (not all inclusive):
    • Disability issues related to private insurance claims and services
    • Characteristics of disability and relationship to ADL, current and future impact on family, social success and referrals to partnering programs
    • How to meet the needs of families with various ethnic, racial, religious backgrounds, age or disability status
    • Trends related to disability supports and interventions that may address the needs of the member
    • Exploration of Social Determinants of Health related areas
    • How to ask probing, disability sensitive questions that enable the Family Advisor to offer care coordination or social services support
  • Utilize critical thinking paradigms in collecting relevant information
  • Participate in projects and initiatives that expand support to members
  • Conduct thorough research into resources for members
  • Drive pro-active work to support members with expected needs for future resources
  • Collects and shares information related to trends of barriers for our members; helps identify and problem solves alternatives to barriers
  • Self-directed work. Expected to identify gaps in advisor knowledge base and proactively engage with advisors to close their gaps
  • Find and build relationships with social and community resources. Act as a resource to Family Advisors to provide these resources that match family needs
  • Respond to requests in a timely manner
  • Participate in regular staff meetings and huddles within Care Specialist team as well as meetings and huddles throughout the FEC
  • Participate in regular professional development activities and supervision
  • Facilitate training and professional development for the FEC leaders and advisors
  • Advocate for spiritual care/person-centeredness in each professional interaction
  • Maintain accurate records of member- and staff-facing care activity to consistently communicate with appropriate members and through the FEC
  • Coordinate with the other Care Specialists and FEC staff to ensure consistent communication and process
  • Create didactic content as Subject Matter Expert in role-related arenas, such as: diversity & inclusion, communication, spirituality, end of life, grief, compassion, etc.
  • Actively participate in the delivery of spiritual care adhering to the Standards of Practice of a nationally recognized certifying body and the principles and practices of Outcome Oriented Chaplaincy

Required Qualifications:

  • Master’s Degree in medical or social science field or equivalent experience or Master’s degrees in chaplaincy-related studies and Board-certified as a chaplain through a nationally recognized chaplaincy certification body.
  • Must be 18 years of age or older
  • 3 years of experience working directly with or on behalf of the pediatric population, specifically with disabilities, in a professional capacity or minimum of 1600 hours of Clinical Pastoral Education (accredited by the United States Department of Education).
  • Experience working with patients diagnosed with complex conditions including, mental and behavioral health conditions, end of life and life limiting disease processes, etc.
  • Experience providing expert and nuanced remote spiritual care via digital platforms such as Microsoft Teams and telephonically
  • Ability to work Monday - Friday between the hours of 7:30AM - 6 PM, CST for a 40-hour work week
  • Experience with Microsoft Word (create and edit documents), Microsoft Outlook (manage calendar, send / receive emails), and Microsoft Excel (spreadsheet data entry)
  • Objectivity and flexibility to work with people with all or no-faith backgrounds.

Preferred Qualifications:

  • Licensed Master's degree-level clinician in Psychology, Social Work, Counseling or Marriage or Family Counseling OR a Licensed Ph.D. level Psychologist OR Psychiatric Nurse. License must be independent and must be active and unrestricted or Three – Five years of experience in professional chaplaincy with more than 200 hours of additional continuing education
  • Experience navigating NORD (National Organization for Rare Disorders) database
  • Knowledge of the public sector, i.e., Medicaid
  • 1 years of experience coaching and / or developing others

Telecommuting Requirements:

  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

  • Focus care and interventions on the care recipient’s frame of reference and model of meaning-making. With the care recipient, they assess and provide interventions to enhance perceived quality of life (QoL) and overall sense of peace.
  • Ability to self-manage with efficiency to navigate ongoing responsibilities, requests, presentations, etc.

California, Colorado, Connecticut, Hawaii, Nevada, New Jersey, New York, Washington or Rhode Island Residents Only: The salary range for this is $58,300 - $114,300 per year. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
