United Way of Harrison and Doddridge Counties is Hiring a Case Manager Near Clarksburg, WV

JOB TITLE: ResCo Professional Case Manager (Mental Health, Substance Use Disorder,

Police and ALICE Response)

JOB DESCRIPTION:

The Resilience Collaborative (ResCo) of United Way of Harrison and Doddridge Counties is

seeking a compassionate and dedicated Professional Case Manager to join its staff. As a

ResCo Case Manager, you will play a crucial role in assisting local law enforcement and

United Way with mental health response, substance use disorder response, WV 211 referrals,

and ALICE (Asset Limited, Income Constrained, Employed) case management intervention.

Your primary responsibility will be to provide integrated case management support and link

residents to community resources to improve their overall well-being.

RESPONSIBILITIES:

● Collaborate closely with the Harrison County Sheriff's Department and the Resilience

Collaborative of United Way to provide mental health and substance abuse response

care and intervention, ensuring the safety and well-being of individuals in crisis

situations.

● Conduct thorough assessments and develop individualized care plans to address the

unique needs of each client.

● Identify and build rapport with individuals experiencing homelessness and/ or living

with Substance Use Disorder throughout Harrison County and work to promote

recovery and housing options.

● Enter client-level data of services into the Homeless Management Information System

(HMIS) in real-time, or within 48 hours of contact or service provision to ensure

proper documentation of client interactions, progress, and referrals.

● Refer and divert individuals from the homeless service delivery system, as applicable.

● Accept referrals from the WV Balance of State Coordinated Entry System, which may

include screening and assessing all potential participants using the Vulnerability

Index-Services Prioritization Decision Assistance Tool.

● Actively engage with the Coordinated Entry System, the Region 1 Housing

Prioritization Team, and the Harrison County Homeless Services Multi-Disciplinary

Team.

● Engage individuals who are experiencing homelessness and living with SUD

following arrest and during incarceration to explore and facilitate treatment and

housing options upon release.

● Assist individuals in connecting with addiction recovery treatment options.

● Maintain a strong knowledge of community resources and actively engage in

networking to stay updated on available services.

● Provide crisis intervention and de-escalation techniques when necessary, ensuring the

safety of both clients and yourself.

● Participate in training and professional development opportunities to enhance your

skills and knowledge in mental health response care.

○ Cultural competency, motivational interviewing, mental health first aid,

assertive engagement, suicide prevention, trauma-informed care,

person-centered care, use of the VI-SPDAT, SPDAT, and HMIS, Critical Time

Intervention, housing procurement and support, as well as other United Way

required trainings.

● Collaborate with other social service agencies, community partners, and

organizations to develop a comprehensive network of support for clients.

● Uphold the highest level of professionalism and ethical conduct in all interactions

with clients, colleagues, and community members.

● Performing such other duties as assigned.

PHYSICAL DEMANDS/WORK ENVIRONMENT:

The physical demands described herein are representative of those that must be met by

prospective candidates to successfully perform the essential functions of this job:

● Extended periods of sitting and or standing, telephone work and/or computer work,

as well as interactions with clients and volunteers.

● While performing the duties, this position is regularly required to sit, talk, and/or hear.

Must be able to remain stationary for up to 50% of the time. He/she is frequently

required to use hands to handle, or feel. He/she is required to reach with hands and

arms. Occasionally required to climb, stoop, kneel, crouch, and/or crawl in confined

spaces. Must be able to lift over 25 pounds repeatedly throughout a shift without

assistance.

● The noise level in the work environment is usually moderate.

● Must have the ability to see and respond to potentially hazardous situations.

● Must wear personal protective equipment (PPE) when deemed appropriate.

● Reasonable accommodations may be made to enable individuals with disabilities to

perform the essential function.

QUALIFICATIONS:

● Must be a self-starter capable of functioning effectively in an independent

environment and on a small team.

● Bachelor’s or master’s degree in social work or a related field.

● Experience in mental health response care, crisis intervention, or similar roles

preferred.

● Experience or willingness to be trained in Mental Health First Aid, Motivational

Interviewing, Assertive Engagement, and Trauma-Informed Care.

● Must be detail oriented, well-organized, adaptable and capable of maintaining

composure when faced with adverse conditions.

● Strong communication and interpersonal skills, with the ability to build rapport and

establish trust with diverse populations.

● Ability to work effectively in a fast-paced, high-pressure environment.

● Willingness to participate in ongoing training and professional development

opportunities.

● Physical ability to lift more than 25lbs and meet the physical requirements of the job.

● Must pass a background check.

● Driver’s license and transportation required.
