JOB DESCRIPTION

Under the supervision of the Clinical Director, the Care Coordinator will have excellent organizational and administrative skills. Care Coordinator is expected to be decisive leaders with superb time management and record keeping skills. To ensure success, Clinical Coordinators should be confident and professional administrators with fantastic communication and IT skills that will contribute to the efficient day-to-day activities of the clinic. The Coordinator works comfortably under pressure while multi-tasking in a fast-paced environment and can effectively react to emergency situations.

MAJOR DUTIES & RESPONSIBILITIES:

  • Responsible to coordinate and manage the clinical services provided by the Mental Health Staff 
  • Maintain the confidentiality of patient information in accordance with the Prestige policies, the standards of the American Psychological Association, National Association of Social Workers and Department of Behavioral Health.
  • Allocate staff resources in mental health services to ensure that consumer needs for clinical and consultative services are met in a timely and professional manner
  • Supervise and monitor mental health services’ staff to ensure compliance with the health care policies and procedures, the policies and regulations of the facility; the responsibilities/duties of their specific position descriptions and the employee policies and procedures of the facility
  • Coordinate in-service opportunities to mental health services staff to ensure compliance 
  • Ensure patients’ clinical documentation and department files are maintained appropriately
  • Responsible for maintaining and coordination of clinic supplies to include inventory control 
  • On-site coordination of psychiatric services by provision of administrative support and patient monitoring
  • Communicate concerns to the Clinical Director and/or his or her designee in a timely manner with concerns which may have a significant impact on an individual patient or on the mental health services program
  • Updates all Treatments Plans under the Supervision of Clinical Manager. Monitor Status Reports from Credible i.e. Local Dollar & Medicaid
  • Monitors and reports treatment plan expiration dates
  • Ensures all Diagnostic Assessments under the supervision of Clinical Manager remain up to date
  • To maintain current and active utilization case load spreadsheet of all CSWs on a weekly basis
  • Maintain all CSW case assignments are up to date 
  • Assist Consumers in conjunction with Community Support Worker to ensure that all consumers apply for Medicaid
  • Coordinates Team Meetings for Consumers in efforts to provide comprehensive services
  • Works closely with Billing team to ensure that all documents are completed in efforts to ensure authorization of services
  • Keep compliance with employee standards of the clinic: this includes, but is not limited to, the maintenance of a working environment and personal appearance consistent with professional responsibilities, development of harmonious working relationships, and timely notification of supervisory personnel of absences from clinic 
  • Must be able to obtain and maintain appropriate licenses in accordance to the rules and regulations of the Department of Behavioral Health in Washington, DC
  • Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
  • Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations
  • Perform other duties as assigned
  • Develop and accurately maintain multiple calendars and event coordination
  • Develop and adjusts coordination structure to ensure fit with operational realities
  • Assist the clinical staff with recertification or quarterly report activities and care coordination efforts as needed and appropriate
  • Assist in efforts to track and monitor progress with adhering to DHCF regulations pertaining to Prior Authorizations, proper documentation, proper discharge, etc.
  • Help facilitate and participate in all meetings and in-service trainings scheduled
  • Available for emergency visits to recipients’ homes to assist with needed documentation or urgent situation
  • Provide training and orienting new clinical staff as needed 

REQUIRED EDUCATION AND EXPERIENCE 

  • High School Diploma or equivalent required, Bachelor’s Degree highly preferred
  • 1-3 years MHRS experience is required 
  • 2 – 4 years of direct community support or mental health resource support 
  • 2 years of clinic coordination and administration of mental health service delivery systems at a healthcare facility 
  • 1 year of professional experience of mental health counseling, group and individual psychotherapy, diagnosis and treatment of major mental disorders and/or psychological evaluation techniques
  • Knowledge of the Consumer Culture 
  • Knowledge of MHRS Systemic Infrastructure for Optimal Service Delivery
  • Current CPR certification
  • Experience with Electronic Medical Record Systems such as Credible, GAINS, LOCUS and/or ICAM’s highly preferred 
  • Experience working with DC Government community resources, agencies and/or social services
  • Working knowledge of medical terminology, HIPAA regulations, ICD and CPT coding highly preferred

WORK ENVIRONMENT

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. 

POSITION TYPE AND EXPECTED HOURS OF WORK

This is a full-time position. The days and hours will vary based on the needs of the clinic.

TRAVEL

Washington DC and Maryland offices occasionally and/or as needed.
