Job Summary:

• Performs patient care activities and other tasks as assigned to assist the primary care, therapy, and adult day health staff. Works under the direct and indirect supervision of the Participant Care Manager but is independent in the application of knowledge and skills.

Required Qualifications

• High school diploma or general education diploma
• Must be CPR Certified 
• Minimum of one (1) year working with the frail and elderly 
• Successful completion of Thome PACE's PCA program
