POSITION SUMMARY:
As the Clinical Consultant, you are responsible for facilitating services that will assist clients to gain access to necessary medical, social, educational, and other services related to their accepted condition(s). The Clinical Consultant ensures adherence to established clinical policies, protocols, regulations, and standards.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Collaborate with health care professionals and service providers to ensure optimal patient care.
  2. Admit patient and transfer responsibility to the local office team.
  3. Prepares sites for survey through survey readiness process.
  4. Maintain departmental policies, procedures, objectives, or patient care standards, based on evidence-based practice guidelines or expert opinion.
  5. Stay current on nursing industry protocols.
  6. Provide consultation to health care providers relating to patient admission, care, and clinical procedures.
  7. Teaching and collaborating with individuals and/or groups on clinical topics including taking the role of Clinical expertise for their locations and assigned region.
  8. Implement and evaluate standards of nursing practice in specialty areas, such as oncology, pulmonary, and geriatrics.
  9. Maintain departmental policies, procedures, objectives, and infection control standards.
  10. Make clinical recommendations to physicians, other health care providers, insurance companies, patients, or health care organizations.
  11. Develop treatment plans, making recommendations to treating physician based upon patient assessment.
  12. Plan, evaluate, or modify treatment programs, based on information gathered by observing and interviewing patients or by analyzing patient records.
  13. Provide specialized direct and indirect care to patients within a designated specialty, such as geriatrics, neurology or oncology.
  14. Monitor or evaluate medical conditions of patients in collaboration with other health care professionals.
  15. Present clients with information required to make informed health care and treatment decisions.
  16. Participate in clinical research projects, such as reviewing protocols and patient records, monitoring compliance, and meeting with regulatory authorities.
  17. Participate in patient education programs that include information required to make informed health care and treatment decisions.
  18. Assist with patient safety issues and education.
  19. Prepare reports to document patients' care activities.
  20. Write nursing orders.
  21. Lead region in developing and installation of vigorous QAPI program.
  22. Supervise quality metric analysis and conduct extensive monthly chart audits for locations with a specific focus.
  23. Conduct training of new intake nurses.
  24. Teach patient education programs that include information required to make informed health care and treatment decisions.
  25. Maintain confidentiality of all office and client records per HIPAA and Privacy Act regulations.
  26. Performs other duties as assigned.

OTHER DUTIES AND RESPONSIBILITIES:

This position may require occasional traveling between locations in your personal vehicle

SUPERVISORY RESPONSIBILITY:

This position has no supervisory responsibilities.

Qualifications


PREFERRED QUALIFICATIONS:

  1. Active Registered Nurse license in the states that services will be provided. (Bachelor’s degree preferred)
  2. Minimum 2 years’ experience in geriatrics, medical, oncology, pulmonology, or equivalent field
  3. Experience working in the home health care industry or familiarity with home health care patients
  4. Knowledge of relevant regulations and guidelines, such as HIPAA
  5. Strong attention to detail
  6. Excellent written and verbal communication skills
  7. Ability to work independently and as part of a team
  8. Excellent organizational and time management skills to meet deadlines and handle multiple tasks simultaneously
  9. Proficient in Microsoft Suite
  10. Must pass a criminal background check

WORK ENVIRONMENT and PHYSICAL REQUIREMENTS:

  1. Ability to sit at a computer terminal for an extended period of time.
  2. While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone, keyboard, reach, stoop and kneel.
  3. Specific vision abilities required by this job include close vision requirements due to computer work.
  4. Regular, predictable attendance is required as business demands dictate.
