Columbus Colony for Elderly CareAs an MDS (Minimum Data Set) Coordinator at Columbus Colony for Elderly Care, you will play a pivotal role in ensuring accurate and comprehensive assessment of resident care needs and regulatory compliance. Your primary responsibility will be to coordinate the completion and submission of the MDS assessments and related documentation to support resident care planning and reimbursement processes.

Supervisory Responsibilities:

  • None.

Duties/Responsibilities:

  • Coordinate the completion of resident assessments using the Minimum Data Set (MDS) tool in compliance with state and federal regulations.
  • Collaborate with interdisciplinary team members, including nurses, therapists, social workers, and physicians, to gather information for MDS assessments.
  • Conduct comprehensive resident assessments upon admission, quarterly, and as needed for significant changes in condition.
  • Review medical records, care plans, and other documentation to ensure accuracy and completeness of MDS assessments.
  • Enter assessment data into the facility's electronic health records system and transmit MDS data to the appropriate regulatory agencies within required timelines.
  • Participate in care planning meetings to develop individualized care plans based on MDS assessment findings.
  • Provide education and training to staff members on MDS documentation requirements and coding guidelines.
  • Monitor and track quality indicators and outcome measures related to resident care and MDS assessments.
  • Serve as a resource for staff members regarding MDS coding, reimbursement, and regulatory compliance issues.
  • Stay informed about changes in MDS regulations and guidelines and implement updates as necessary.

Required Skills/Abilities:

  • In-depth knowledge of MDS regulations, guidelines, and reimbursement systems.
  • Proficiency in electronic health records systems and computer skills.
  • Strong analytical and critical thinking skills, with attention to detail.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary team members.
  • Ability to prioritize tasks, manage time efficiently, and meet deadlines in a fast-paced environment.

Education and Experience:

  • RN or LPN license in OH required.
  • Minimum of 5 years of experience in long-term care or skilled nursing facility setting, with prior experience in MDS coordination preferred.
  • Certification in MDS assessment (RAC-CT) preferred.

Physical Requirements:

  • Ability to stand, walk, and sit for extended periods.
  • Manual dexterity to operate computer equipment and input data accurately.
  • Visual acuity and auditory ability to review medical records and communicate effectively.
  • Lift and carry up to 20 pounds of materials or equipment as needed.
