JOB TYPE: Full-time
PAY RANGE: 45,000-55,000 per year

LOCATION On-site in Saint Paul, MN 55103; Must be able to reliably commute or plan to relocate before starting employment.
PHYSICAL SETTING Emergency shelter/transitional housing environment
SCHEDULE
  • 8-hour shift
  • Will include some evenings
  • Tuesday to Saturday
MINIMUM JOB REQUIREMENTS
  • Bachelor’s degree in human services or social work preferred
  • Knowledge of the principles and practices of case management
  • Knowledge of county, Department of Human services, and permanent supportive or permanent housing resources
  • Knowledge of available Veterans resources and programs
  • Experience working with men experiencing homelessness and poverty
  • Respectful of cultural diversity and the ability to interact positively with clients, other staff members and agencies within the community
  • Emotionally mature and able to work in high-pressure situations
  • Solid personal boundaries and ability to maintain a healthy personal and professional life balance
  • Mature in Christian faith and knowledge of, understanding of, and ability to demonstrate and express grace and mercy in interactions with program residents
  • Must be in agreement with Union Gospel Mission Twin Cities’ Statement of Faith
QUALIFICATIONS & CERTIFICATIONS
  • Bachelor’s degree (Preferred)
  • Case management experience (Preferred)
  • Valid driver’s license (Preferred)
  • U.S. work authorization (Required)
  • Proficient in the Microsoft Suite (Word, Excel, PowerPoint, Outlook)

UGMTC’s Men’s Programs serve men 18 years or older who are experiencing hunger, homelessness and addiction. Programs include Emergency Services, Christ Recovery Center (CRC), the Discipleship Program, Housing Plus and the Veterans Housing Program (Pilot). Men who come to UGMTC are offered food, shelter, clothing, mental health services, support services, education, job-skills training and Christ-centered spiritual support so that they can make progress toward healing, stabilize their lives and experience independent living.
We are hiring a Case Manager, Housing Plus/Veterans Plus Program to be an integral part of the Housing Plus (transitional housing program) and Veterans Plus programs in supporting men and male Veterans experiencing homelessness and addiction with dignity and compassion. POSITION SUMMARY
This position provides direct case management services for men who are a part of our Housing Plus/Veterans Plus programs in alignment with UGMTC’s mission, values, goals and outcomes. Reporting to the Housing Plus/Veterans Plus Program Manager, this role will collaborate with a highly dedicated team committed to exceptional care.
DUTIES AND RESPONSIBILITIES
  • Conducts a needs assessment screening for new residents to obtain information for formulating an appropriate individual plan with reasonable, achievable goals.
  • Maintains a caseload of residents meeting monthly. Conducts crisis counseling when needed and recommends appropriate treatment or action. Ensures program residents meet their assigned goals and adhere to program policies.
  • Performs drug/alcohol screening of men as requested.
  • Maintains an atmosphere on the floor that is conducive to safety, dignity and cleanliness.
  • Facilitates groups or teaches classes as assigned.
  • Attends and participates in Mission and department meetings and training as required.
  • Provides ongoing case management. Maintains each resident’s file; prepares complete and accurate case notes; documents each visit and all occurrences. Maintains records of case management encounters for monthly statistical reports.
  • Prepares written correspondence and reports; may prepare statistical reports and summaries.
  • Attends weekly staff meetings. Contributes to the efficiency and effectiveness of the Housing Plus/Veterans Plus service to its program residents by offering suggestions and participating as an active member of a resident’s care team.
  • Maintains cooperative relationships and communication within program team, other departments and external constituents to maintain an environment conducive to idea exchange and support for the achievement of project objectives.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
  • Well-developed interpersonal communication and customer service skills to work effectively with a team, as well as in a wide range of constituencies in a culturally diverse community
  • Proficiently skilled with Microsoft Office applications to enter data, track, compile and report on program statistics
  • Ability to read, understand and follow safety and other instructions and procedures
  • Knowledgeable in and practices the skills of crisis intervention, problem resolution and conflict resolution
  • Ability to maintain residents’ confidentiality in accordance with all policies and applicable laws
  • Committed to serving those experiencing homelessness, hunger and addiction
WORKING CONDITIONS & PHYSICAL EFFORT:
  • Work is performed in an emergency shelter/transitional housing environment and involves physical effort encountered in normal, everyday office activities.
  • Must be able to lift up to 40 lbs.
