Qualifications


JOB SUMMARY: The Wellness Circle Coordinator is responsible to design, implement and coordinate effective wellness programs for disease management and prevention.

SPECIFIC JOB DUTIES:

  1. Serves as point of contact and administrative expert on Centering and Wellness Circle programs.
  2. Promotes programs organizationally and externally and recruit patients and schedules appointments for Centering and Wellness Circle groups.
  3. Prepares for each session by completing or assisting with program set up. (Prepare documents, supplies and schedule/secure and set up room space, etc.)
  4. Collaborates with clinician to develop wellness group agenda and activities.
  5. Serves as clinical support and presents and participates in group activities.
  6. Assists in training of staff for Centering and Wellness Circle programs.
  7. Collaborate within Union and with community partners to ensure optimal utilization and continuous program improvement.
  8. Other duties as assigned.

POSITION REQUIREMENTS:

  • Certified Medical Assistant
  • Excellent telephone and customer service skills.
  • Exceptional teamwork and interpersonal skill.
  • Prior experience in primary care, preferable with strong pediatric or prenatal care background.
  • Attention to detail and organizational skills.
  • Ability to sit for at least one hour at a time.
  • Ability to speak, read and write English fluently.

ESSENTIAL FUNCTIONS:

In order to fulfill the requirements of this position, duties 1 through 8 are considered essential functions of the job.

Physical Demands:

  • Ability to lift up to 20 lbs.
  • Intermittent physical activity including walking, standing, sitting, lifting and supporting patients.
  • Exposure to virus, disease and infection from patients and specimens in working environment.
  • Exposure to blood and/or body fluids.
  • Ability to work in a stressful environment.
  • Scheduled based on operational need. (rotating shifts)
