Job Summary:

The Remote Patient Re-Engagement Specialist will reconnect with former Uni Care Home Health Medicare patients. The role involves using a structured protocol for calls, assessing patient needs, offering UniCare services, and ensuring compliance with healthcare regulations. Hourly rate is $20.00.

Key Responsibilities:

• Conduct patient re-engagement calls using a detailed script.

• Assess patient health status and needs, offering appropriate UniCare services.

• Handle language barriers and diverse patient demographics.

• Maintain strict compliance with HIPAA and other healthcare regulations.

• Document all patient interactions accurately for follow-ups.

• Collaborate with healthcare team and report activities to the Sales Manager.

• Monitor and evaluate program effectiveness continuously.
