UNC Health Appalachian is Hiring a Care Mgmt Assistant Near Boone, NC

Position Summary
  • The Care Management Assistant supports the functions of the Care Management Department by performing a wide variety of administrative duties including but not limited to data entry, report generation, sending referrals, answering phones, scheduling follow up appointments, initiating pre-authorization for patient placement, and assisting with patient completion of necessary forms or applications. Performs other duties as required.
  • The Care Management Assistant integrates knowledge and skills with effective interactions to provide quality patient care and useful diagnostic information. Provides direct and indirect ways to communicate with those who need it, meeting the needs of each encounter, in-line with their scope. Decisions and actions with patients and families are determined in an ethical manner practicing compassion and respect for the inherent dignity, worth, and unique attributes of each person. Effectively communicates with patients/families, visitors, colleagues, physicians, leadership, and volunteers. All areas of work are provided to achieve and maintain top-decile results and outcomes for patient experience, quality, and patient safety.
  • All areas of work demonstrate the UNC Health Appalachian's Mission, Vision, and Values in action and approaches work in a way that reflects action, efficiency, and personal accountability. Demonstrates professional conduct, effective communication, and effective completion of tasks. Effectively and professionally manages working relationships with coworkers, physicians, and all UNC Health Appalachian team members in a way that promotes a healthy work environment. Responds quickly and accurately to emergencies in the work environment. Performs duties under direct supervision. Always assists coworkers, patients, families, and visitors regardless of the patient assignment, so long as the assistance is in-line with their scope.
    • Assists the Care Management department with daily workflow and process improvement. Assists both RN Case Managers and Social Workers/Discharge Planners by absorbing tasks which do not require RN licensure or social work specialty.
    • As assigned, administers second Important Message from Medicare (IMM) and Medicare Outpatient Observation Notice (MOON) and documents accordingly. Creates and maintains a means of organizing required and completed regulatory forms to assist the department with compliance.
    • Assists in facilitating discharge planning by offering administrative support including but not limited to agency or resource referrals, initiating, and following up on pre-authorization required for patient placement to SNF, and arranging transportation. Escalates potential barriers to discharge to the case manager/social worker as identified.
    • Schedules appropriate follow-up appointments for patients, considering and problem-solving any identified barriers to post-discharge compliance.
    • Processes and monitors incoming and outgoing faxes in a timely manner. Maintains supply inventory. Picks up and delivers departmental mail and coordinates mailing of department documentation.
Position Qualifications
  • Minimum Education
    • High School Diploma or GED is required.
    • Advanced coursework in business or medical terminology is preferred.
  • Experience
    • Two (2) years of prior experience in a medical or office setting is preferred.

    • 5. Ability to work independently, self-motivated.
    • 6. Effectively supports organizational goals and objectives, mission, vision, and core values through appropriate use of interpersonal skills and job performance.
    • 7. Demonstrates well-defined organization and time-management skills.
    • 8. Identifies, analyzes, organizes and solves problems in an efficient and timely manner and collaborates with others to make sound, timely decisions.
  • Physical Demands
    • Manual dexterity and mobility.
    • Prolonged, extensive, or considerable sitting and, at times, standing/walking.
    • Bending, squatting, twisting, turning.
    • Near acuity: ability to see clearly at 20 inches or less.
  • Working Conditions
    • Regularly exposed to the risk of blood-borne diseases.
    • Frequently exposed to hazards from electrical/mechanical/power equipment, odorous chemicals and specimens, and unpleasant elements (accident, injury, slippery surfaces, uneven surfaces), etc.
    • Subject to irregular hours (days, evenings, nights, weekends, holidays) and emergencies.
    • Continuous sitting or standing most days >8 hours per day.
    • Episodes of walking/standing greater than >6 hours per shift.
    • Speaking, reading, and writing > 8 hours per day.
    • Needed to frequently: lift, carry, push and/or pull >50 lbs.
    • Frequently per shift: bends, squats, kneels, twists, turns, and reaches.
    • Frequently per minute: hand, finger, and wrist repetition and moderate dexterity.
    • Frequently per hour, exposed to soft voices > 3 ft away. Frequently per hour, exposed to close eye work.

Benefits and Compensation

Compensation is commensurate upon education & experience. Benefits begin the first of the month following 60 days of employment from the new hire orientation date. This position is a non-exempt position.
