In this position, the Licensed Practical Nurse (LPN) will be responsible for preparing the patient for examination and managing individualized patient care by promoting and restoring patient’s health through the nursing process. Collaborating with physicians and multidisciplinary team members and providing physical and psychological support to patients. The LPN will assess patient health problems, administer nursing care to patients, and maintain vital medical records in an Urgent Care setting. The LPN will also play a role in the clinic’s goal to achieve and maintain Patient Centered Primary Care Home (PCPCH) recognition and optimizing electronic health records and other Health Information Technology. The LPN will assist in implementing nursing care plans and advise patients on health maintenance while providing guidance and preventative care that is within the Scope of Practice of the LPN.

Your Impact:

  • Conduct comprehensive patient assessments, including vital signs, medical history, and current condition.
  • Assist physicians and Registered Nurses (RNs) in developing and implementing individualized care plans for patients.
  • Administer medications and treatments accurately and in accordance with physician orders and established protocols.
  • Monitor patient progress and response to treatments, reporting any changes or concerns to the healthcare team.
  • Provide patient education and support to promote wellness and self-care management.
  • Collaborate with other healthcare professionals to ensure continuity of care and optimal patient outcomes.
  • Document all patient interactions, assessments, and interventions accurately and in a timely manner.
  • Adhere to infection control protocols and maintain a safe and clean patient care environment.
  • Respond promptly to emergencies and provide appropriate interventions as needed.
  • Uphold patient confidentiality and privacy in accordance with HIPAA regulations.
  • Participate in ongoing professional development activities to enhance nursing skills and knowledge.
Your Credentials:
  • Current LPN License issued by the State of Oregon.
  • CPR Certification.
  • Demonstrated ability to perform basic nursing skills that are within the LPN Scope of Practice. 
  • Must be able and willing to work 3 12-hour shifts, including some weekends.
  • Proficient knowledge in EHR and computer skills including MS Office suite preferred; typing and 10-key.
  • No suspension/exclusion/debarment from participation in federal health care programs (e.g. Medicare/Medicaid).
