About the Black Visions of Wellness Program

The UMMA Community Clinic Black Visions of Wellness (BVOW) program is designed to support African and African American communities by providing access to traditional and non-traditional care. We strengthen African and African American communities through services that encourage physical health, mental well-being, and recovery while bringing cultural awareness and appreciation of the mind, body and soul. We aim to address chronic health conditions, substance use problems, trauma, depression, and stress. SUMMARY

Under the supervision of the BVOW Program Manager, the Chronic Care Case Manager/LVN will provide coordination and tracking of medical services to enrolled clients (.50 FTE) as well as outreach, engagement and education to enrolled clients and in the community (.50 FTE).

Essential duties

In this role, you'll be responsible for evaluating patients' treatment plans, monitoring patient well-being, creating case progress reports, and ensuring patients are well informed regarding their care and treatment options.

  • Excellent knowledge of case management principles and healthcare management.
  • Ability to work with patients with disabilities, mental illnesses, or challenging behaviors.
  • Good communication and interpersonal skills.
  • Ability to be compassionate, empathetic, patient, and calm when working with patients.
  • Solid organizational skills and a high level of attention to detail.
  • Ability to remain up-to-date with emerging medical knowledge and technology.
  • Provide diverse, complex and confidential clinical coordination of ISM Enrolled Clients.
  • Provide individual and group health education on chronic medical conditions to enrolled clients and in the community.
  • Participate in, and provide medical updates during weekly case conferences.
  • Identify and submit needed medical supplies to support work with clients and outreach and engagement activities.
  • Identify and implement outreach, engagement and education activities to recruit clients to program.
  • Provide monthly reports on case management services and outreach and education activities.
  • Collect, track and input medical outcome data in EHR system
  • Demonstrate patterns of continued learning in order to ensure growth as a practitioner.
  • Participate in Quality Improvement as required.
  • Perform other duties as required by supervisors (and providers when appropriate).
  • Perform basic clinical related duties as necessary.

SUPERVISORY RESPONSIBILITY

The incumbent in this position does not have supervisory responsibilities.

POSITION REQUIREMENTS

To perform effectively in this position, the incumbent must have:

QUALIFICATIONS

1. Graduate from an approved nursing program with degree, or equivalent degree

2. California License as a Licensed Vocational Nurse or interim permit for LVN

3. Current CPR/BLS Certification

4. Must have knowledge through practical experience and/or training in the areas of:

  • Public Health Nursing
  • Clinic Nursing

5. Must be aggressive in developing of patient care programs within the clinical policies and programs

objective and relating them to the funding requirements

6. Must be sensitive to the needs of the patients in a community health care center setting, in the areas

of health and social services as one component

The environment for this position is a medical clinic that is mostly clean and comfortable. It may include some minor annoyances such as noise, odors, drafts, etc. The incumbent is in a non-confined setting in which he or she is free to move about at will. In addition, the incumbent may travel via personal vehicle or public transportation throughout the community.

PHYSICAL ACTIVITY

In the course of performing this work, the incumbent:

  • Will spend substantial time standing, sitting, speaking, writing and listening
  • Must occasionally lift and/or move up to 10 pounds, and occasionally lift and/or move up to 25 pounds
