Under the direction of the center’s Clinical Chief and the Principal Investigator for the Mass DPH (TB and Harm Reduction) contracts, triages phone calls, provides follow-up instructions, coordinates initial and follow-up care in the TB and AIDS Project Worcester (APW) clinics to provide services as required.
- Provide medical case management for patients with TB infection and risks of blood borne and sexual transmitted infections (STD’s). This includes but is not limited to: obtaining and reviewing medical records for new patients, ensuring patient information is collected and documented, assisting medical and mid-level providers when required, and ensuring that applications for additional patient services such as transportation and supplemental insurance are completed and monitored.
- Orient new patients to the TB Getchell Ward, HIV, and Harm Reduction Programs. Provide intensive one-on-one instruction regarding the importance of adherence to regimens.
- Monitors patient medication adherence, lab results, and scheduling of appropriate follow-up appointments. Makes reminder calls and follows up if patient does not keep appointments. Prepares prior authorization requests and calls in prescriptions as authorized.
- Educates patients and families about TB and HIV disease, transmission and medications, positive prevention techniques and appropriate medical follow-up. Monitors adherence to treatment plan.
- Maintains records of patient contacts and appointments in accordance with guidelines.
- Communicates patient issues on a regular basis to include, but not limited to, other team members, staff at Getchell Ward TB Program and AIDS Project Worcester, and referral agencies.
- Reviews educational materials about TB, HIV, and harm reduction for dissemination to patients, other providers and the public. Represents the program by participating in outreach activities such as health fairs, immunization clinics and local cultural events.
- Maintains current information about related diseases through attendance at meetings, interaction with other staff, online trainings, and self-directed learning.

Position Qualifications:
Required:
- Licensed Practical Nurse.
- Current registration to practice as a licensed practical nurse (LPN) in the Commonwealth of Massachusetts.
- Registered Nurse.
Required:
- Minimum of 3 years’ nursing experience.
- Proven work experience in a team environment.
- Must have demonstrated ability to function independently.
- Ability to communicate effectively both verbally and in writing with patients, the public and medical staff.
- Knowledge of, or willingness to learn about, TB, sexual transmitted or blood borne infections and commitment to working with others to decrease risk of new infections.
- Working knowledge of Microsoft Office products (Outlook, Excel, Word etc.).
- Valid Massachusetts driver’s license and reliable vehicle.
- 5 years’ nursing experience.
- Previous clinical TB experience or alternative care sites (example mobile vans, community service agencies).
- Bilingual in English and Spanish.
