CHL's Health Home Management department provides care management to CHL clients who are also enrolled in Commonwealth Care Alliance's OneCare program. We help coordinate these individuals' primary and behavioral health care to ensure they receive high-quality, integrated care.
CHL's Health Home Management department offers staff an opportunity to learn about a variety of topics in health care, including insurance authorizations, long-term home services, primary care, behavioral health, addiction treatment, and hospital care.
Responsibilities:
Assist in the management of CHL Health Home program to ensure compliance with stakeholder contract and program sustainability.
Provide oversight and supervision of assigned staff to include assistance with hiring, training and annual performance reviews.
Help oversee program tracking documents to maintain quality and productivity.
Monitor assigned reports from stakeholder and ensure distribution to the care partner team as needed.
Manage a case load of HHM members as needed and perform all care partner duties for members.
Learn and work with CHL and CCA electronic health records to perform all needed functions and support staff in training, updates, and accuracy of documentation.
Assist with staff meeting minutes and memos.
Perform other related job duties as assigned by the Program Director and Integrated Care Management Director.

Qualifications:
Minimum of a Bachelor Degree in Human Services or a related field required, masters degree preferred.
3 or more years' experience required in healthcare and/or behavioral healthcare; case management or care management
Supervisory or management experience preferred.
Excellent interpersonal skills and customer service skills required.
Good computer skills (Word, Excel, Outlook.), proficiency with Excel and multiple electronic health records
Strong verbal communication skills: attention to detail and organizational skills required.
Ability to work autonomously, be flexible, multitask and be efficient with time management.
Motivated and willing to further professional development through CHL program specific trainings.
Knowledge of homelessness and related issues, familiarity with community resources and entitlements (i.e., substance abuse and mental health).
Current valid US-issued driver's license and a registered inspected and insured automobile for work related purposes including transporting clients in a personal vehicle.
