POSITION SUMMARY:
Cedar Springs Hospital is seeking a dynamic and talented Step-Down Navigator. This position will be responsible for meeting with all eligible patients to discuss the benefits of step down services as part of their continuum of care treatment. Additionally, this role will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to eligibility.

QualificationsKEY RESPONSIBILITIES:
EXPERIENCE:
Minimum of one (1) year of clinical experience in an inpatient psychiatric setting. Thorough understanding of step-down process and familiarity with involved departments required. Thorough understanding of inpatient workflow process, as it pertains to case management and discharging planning preferred.
EDUCATION:
High School diploma or equivalent required. Bachelor's degree in Social Services or Patient Care related field preferred.
CERTIFICATION:
Current CPR, Physical and Verbal Crisis Intervention Certification. Training may be obtained during new hire orientation.
