Responsibilities
This opportunity is with the Home Health Division of Texoma Medical Center. This position covers Cooke County Texas, and is based in Gainesville, TX. Employees are eligible for competitive base rates, benefits, mileage reimbursement, on-call pay, and flexible scheduling. We provide needed supplies.
Primary functions are to administer skilled nursing care for patients in their place of residence, coordinate care with the interdisciplinary team, patients and their families, and a referring agency. Assumes the responsibility for and participates in the coordination of care. Responsible for the case management of the home health patient which includes care coordination; supervision and management of patient care; required documentation; supervision of the LVN and Home Health Aide; and appropriate internal, external and community referrals.
Texoma Medical Center, a 414-bed acute care facility has been providing quality health care to the residents of North Texas and Southern Oklahoma since 1965. Our main campus is located in Denison, Texas, approximately one hour north of the Dallas/Fort Worth area and just south of the Texas/Oklahoma border. Qualifications
Registered Nurse (RN) Job Requirements:

• Current Texas nursing license or compact license.
• One year experience as a nurse in a clinical care setting. One year full time home health experienced preferred.
• AHA BLS required at hire.
• Must successfully pass any pre-employment assessment(s) required by the facility.
• Must be able to identify and competently address needs of populations served.
