The Associate Vice President of Patient Care Transitions will lead and oversee the strategic planning, development and execution of initiatives aimed at optimizing patient care transitions across the health care continuum. The incumbent will play a pivotal role in ensuring seamless transitions of care for patients within UF Health Shands and its affiliated facilities, ultimately enhancing patient outcomes, satisfaction and operational efficiency.

Minimum Education and Experience Requirements:

• Master's degree in health care administration, business administration, nursing or a related field.
• Minimum of 10 years of health care management leadership experience with a focus on care transitions, post-acute care management and care coordination.
• Strong leadership skills with the ability to build and lead high-performing teams.
• Proven track record of developing and implementing successful care transitions programs.
• Demonstrated ability to work collaboratively with clinical and operational leaders across a complex health care organization.
• Excellent communication and interpersonal skills with the ability to communicate effectively with stakeholders at all levels.
• Strong analytical and problem-solving skills with the ability to develop and implement metrics to measure the effectiveness of care transitions.
• Knowledge of health care regulations and policies related to care transitions and post-acute care management.
• Experience working in an academic medical center or other complex health care organization preferred.
• Experience with case management, social work, SNFs, rehab programs, etc., is preferred.
