Plans, develops, mentors and implements in-patient diabetes management and self-management education (DSME) for patients, families and nursing staff. Develops, revises and implements diabetes education curricula and CE offerings according to identified learning needs. Facilitates competency programs that are consistent with a team approach to diabetes management and education. Collaborates with community resources, medical and nursing staff on patients referred for DSME. Participates in the Diabetes Steering Committee, policy and procedures, patient education, nurse councils, quality and performance improvement and other related activities as may be determined and may apply to diabetes management and education. Provides feedback and solutions to opportunities for enhancing staff performance, clinical competence and professional development. Facilitates an environment that values and enhances learning, knowledge acquisition and professional development.
**Monday-Friday- 1st shift**
**Diabetes Management and Self-Management Education along with 1-2 years of teaching experience (Formal or informal) AND 3-5 years of clinical experience.
**Patient education- Adult population**
**Works with multi-level healthcare providers**
**Participates in the Diabetes Steering Committee**
