Role: Experienced Home Care Nurse

Requirements:

• Minimum 2 years of home health nursing experience
• Proficiency in Axxess software
• Strong charting skills
• Valid IL nursing license

Responsibilities:

• Conduct comprehensive assessments in clients' homes
• Develop tailored care plans for optimal outcomes
• Administer medications and provide skilled nursing care
• Monitor patient progress and adjust plans as needed
• Educate patients and caregivers on wellness and condition management
• Maintain accurate electronic records
• Communicate effectively with patients and healthcare professionals
• Ensure compliance with home health regulations
