Job Summary

UC Davis Transplant Center is a UCDH-designated clinical center responsible for clinical management and coordination of solid organ pre- and post-transplant patient care and living donation. The program serves UCDH patients requiring solid organ transplantation and living organ donors.

Minimum Qualifications

  • Graduation with a Master’s degree, DNP or PhD in Nursing from an accredited School of Nursing
  • Graduation from an accredited school of nursing.
  • At least six (6) months of clinical experience.
  • 2 years or greater relevant experience including transplant coordination or critical care experience
  • Nurse Practitioner certified, Nurse Practitioner license without restriction
  • American Heart Association (AHA) BLS or CPR or UCDH-issued CPR Certification
  • Furnishing license
  • DEA within 3 months of hire
  • Ability to read, write, speak, and understand the English language.
  • Ability to utilize medical terminology appropriately.
  • Knowledge of current trends in nursing.
  • Commitment to excellence in patient care.
  • Effective interpersonal skills.
  • Demonstrates accountability and professionalism
  • Adapts to change in a positive, professional and productive manner.
  • Knowledge of legal implications in clinical practice and documentation.
  • Knowledge of performance improvement and evidence-based practice.

  • Ability to assess, plan, implement and evaluate patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients.

  • 2-3 years of relevant experience including transplant coordination, dialysis experience, or critical care experience.
  • CCTC
  • Knowledge and skills related to transplantation including:
  • referral and selection criteria
  • evaluation testing
  • immunology, HLA testing and it’s implications
  • desensitization principles, protocols, and side effects of biologicals
  • coordination of transplant event
  • triage of post transplant patient and live donor phone calls
  • Ability to independently set priorities and make clinical decisions in high paced environment
  • Knowledge of transplant medications, indications, dosage ranges, side effects, and potential toxicity

Department Description

The Transplant Coordinator (TC) NP coordinates care for adult and pediatric patients in end stage organ failure across the continuum of transplant care (pre-transplant, during transplant, and post-transplant), in addition to adults across the continuum of living donation (evaluation, inpatient, post-donation). The TC NP is the process expert regarding transplant phases of care and living donation and educates patients, caregivers and hospital staff about transplant-specific topics. The TC NP collaborates with members of the health care team to enhance patient care and serves as investigator, consultant, and leader. The TC NP makes clinical judgments based upon a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems. The Transplant Coordinator NP is an integral member of a larger interdisciplinary team contributing to excellence in patient care, research, teaching, and in providing leadership to the organization.
