U.S Comm for Refuge is Hiring a Health Wellbeing Case Manager Near Raleigh, NC

The Health and Wellness Case Manager (HWCM) is responsible for direct service provision to refugees, asylees, humanitarian parolees, and other eligible populations including individuals with complex medical and mental health conditions, home visits, health orientation, health care navigation, transportation, etc. The Case Manager will collaborate and work closely with other members of the Preferred Communities team. The HWCM will handle case documentation online and in paper files as well as program reporting. This is a full-time position and reports to the Program Manager of Health and Wellness. This position is funded through September 30, 2024. Continuation of employment is contingent on the availability of ongoing funding.

DUTIES AND RESPONSIBILITIES

  • Support individuals with complex medical and mental health needs and will make appropriate referrals and work to ensure access to appropriate care and services;
  • HWCM will develop and deliver education sessions related to chronic diseases, sexual and reproductive health, nutrition, stress management, etc.
  • Work in collaboration with providers to improve the health status of refugees arriving with complex medical or mental health needs by providing added, focused case management services and care coordination;
  • Ensure that refugees are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary;
  • Cultivate, establish, and maintain relationships with partners, external stakeholders, and support outreach efforts to increase access to healthcare resources for clients;
  • Conduct home visits, intakes and assessments of clients referred to the program;
  • Coordinate appointments and services, maintain case files and case notes for clients enrolled in program;
  • Work together with team members in the Preferred Communities Program and resettlement agency to serve clients with medical needs;
  • Attend meetings of stakeholders in the health community to create new partnerships and share resources;
  • Perform other job-related duties as assigned by supervisor.

REQUIREMENTS

  • Bachelor’s degree in related field, such as (but not limited to) public/global health, healthcare administration, or social services, from an accredited institution;
  • Demonstrated track record of cultural humility and experience working with diverse populations;
  • Excellent written and interpersonal communication skills such as writing reports and materials for different audiences and public speaking;
  • Must work in person at the Raleigh office;
  • Strong skillset in MS Word, MS Excel, MS PowerPoint, and Adobe, preferred skillset in MS Access;
  • Self-starter with the ability to work well in a team, juggle multiple priorities, and meet regular deadlines;
  • Strong attention to detail; must be highly flexible, and able to keep multiple assignments moving forward with a focus on continuous improvement;

PHYSICAL DEMANDS

  • Use of manual dexterity, tactile, visual, and audio acuity.
  • Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands.
  • Occasional lifting (up to 25 pounds), bending, pulling, and carrying.
