Summary

Provides comprehensive care to the assigned patients and serves as direct support staff to Providers.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Provides clinical care as directed by RN, PNP or Physician.

Intake and exit of patients.

Administers immunizations and other medications as ordered by provider and notes time and amount on EMR.

Obtains and records patient's vital signs and weight.

Interviews patient to determine medical problem/condition and documents in chart for provider.

Observes patients and reports adverse reactions to medication or treatment and reports to provider and documents in EMR for assessment and reassessment.

Reviews physician's orders, lab requests, or follow-up needs with patient.

Documents services performed for billing purposes.

Collects samples, such as urine, blood, and sputum, from patients.

Sterilizes equipment and supplies when needed.

Maintains supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies.

Responsible for nursing telephone triage and non-stress test assessment and resulting, responding to patient telephone inquiries by evaluating and responding to the patient’s problems and questions.

Participates in the CPI program for continuous quality improvement and risk management purposes.

Participates in CPI program. Maintains compliance, is knowledgeable, and utilizes the guidelines of all state and federal laws and regulations as they pertain to the position including, but not limited to; Health Insurance Portability Accountability Act (HIPAA), Scope of Practice, Accreditation Standards, Occupational Safety and Health Administration (OSHA), Bureau of Primary Health Care/ Health Resources and Services Administration (BPHC/HRSA), National Committee for Quality Assurance/Patient Centered Medical Home (NCQA-PCMH), Centers for Medicare and Medicaid Services (CMS), Clinical Laboratory Improvement Amendment (CLIA), Joint Commission on Accreditation of healthcare Organizations (JCAHO), and other HRSA related regulatory functions and/or agencies that Family Circle of Care participates in.

Patient Centered Responsibilities

Demonstrates ability to adjust to a varying workload and organize workload to maximize use of time without decrease in patient quality of care.

Participates in team huddles to promote clear communication amongst care team and treating provider.

Provides on-going patient teaching. Documents all care given in EMR.

Follows up with patients on lab, x-ray, or other diagnostic testing results including the scheduling of any additional tests as directed by provider.

Lead Responsibilities

Responsible for assisting Site Manager in day to day operation needs as assigned.

Assists Site Manager with shift absences and coordinating daily workflow.

Assists with schedule requests from staff and provides oversite to Site Manager regarding staff schedules.

Orders vaccines and maintains supply count, lot numbers, and sends VFC report to the state of Texas.

Coordinates the front and back office workflow needs by providing direction and acting as a liaison when needed.

Education and/or Experience

Associate's degree or equivalent from two-year College or technical school in Vocational Nursing; prefer one year clinical nursing experience.

Computer Skills

To perform this job successfully, an individual should have knowledge of Microsoft Word Processing software, Microsoft Excel Spreadsheet software and Electronic Medical Record experience. EPIC experience preferred.

Equipment

Point of Care Testing with glucose meters, and blood pressure monitors and other POCT equipment.

Certificates, Licenses, Registrations

LVN License in the state of Texas required. Responsible to maintain license and obtain all professional CEU's in compliance with Licensure Boards and the City, State and Federal Regulations. Current BCLS Healthcare Card renewed every two years.
