JOB DESCRIPTION POSITION TITLE: HEALTH SERVICES DIRECTOR POSITION SUMMARY: The Health Services Director directly oversees the delivery of care to Assisted Living residents to ensure that the emotional, physical, psychological and safety needs are met through all program services. This includes medication management, staff coordination & training, infection control, coordination with outside healthcare providers, communication with families and applying the nursing process to the care of seniors. Works within State and Federal regulations. REPORTS TO: EXECUTIVE DIRECTOR POSITION RESPONSIBILITIES: A. MANAGEMENT 1. Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for Residents. 2. Identifies ongoing needs and services for Residents through assessment and service planning utilizing the nursing process. 3. Coordinates, in cooperation with the Executive Director, ancillary health care services for Residents. 4. Adheres to all established policies and procedures. 5. Responds to Resident or staff concerns and refers them to the Executive Director when appropriate. 6. Manages and performs TB testing/screening for Residents and staff per state regulations. 7. Monitors quality of work performance of Care Partners, Medication Staff, Licensed Nurses and reports to Executive Director. 8. Assures infection control procedures are known/followed by staff. Directs the infection control measures for the community. 9. Participates in service plan conferences; maintains medication and treatment records for each Resident. 10. Works closely with staff and designated agent(s) to identify Resident problems/concerns/issues; follows up as needed and communicates same appropriately. 11. Has 24/7 overall responsibility for the clinical operations. Is on call according to pre-established schedule. Oversees and/or completes monthly nursing schedule for nursing staff. Participates in the Manager on Duty program as directed by the Executive Director. 12. Coordinates/provides treatments within scope of nursing license and state regulations. 13. Observes all work, safety, and administrative rules. 14. Clinical liaison with families, Residents, healthcare professionals and team members. 15. Manage labor costs, supplies and other line items in the clinical budget. 16. Develop strategies to eliminate/minimize the use of staffing agencies. Keeps the Executive Director apprised of staffing agency usage. 17.Participates in the company quality assurance program. B. RESIDENT SERVICES 1. Conducts initial nursing assessment(s)/evaluations to determine appropriate level of care. This may be done in a prospective Resident’s home or alternative healthcare setting. 2. Conducts reassessments per state/company requirements: this includes significant change of condition, and prior to readmission to the community. 3. Works closely with the Executive Director to counsel the Resident and designated agent(s) meeting personal and related healthcare needs. 4. Evaluate personal services rendered to Residents and coach staff on expectations 5. Evaluates health emergencies and determines emergency medical measures taken regarding Resident care. 6. Acts as Resident advocate in medical-social situations involving Resident and/or designated agents, physician, other agents, or agencies. 7. Coordinates administration of medications and treatments; observes results and documents same. 8. Examines and gives first aid to Residents and staff, within scope of practice. 9. Addresses physical, social, spiritual, and psychological needs of Residents. 10. Completes required forms to ensure continuity of care. 11. Notifies physician and/or designated agent(s) of Resident change in condition. 12. Performs other duties as assigned. 13. Consult with Regional Director of Operations and Corporate Director of Quality & Clinical Services on Resident issues as needed. C. ADMINISTRATIVE 1. Reads staff communications and maintains familiarity with other Resident records as required. 2. Assures accurate transcription of physician orders into treatment/medication record. 3. Assists as directed with move-in and orientation of new Residents and designated agent(s). 4. Reviews Resident service records, treatment and medication books to assure quality of care and documentation in the medical record is within nursing standards. 5. Verifies physician orders, move-in/move-out information and initiates appropriate action/followup. 6. Supervises ordering of medications from pharmacy. 7. Coordinates therapeutic diet orders with staff as required. 8. Assures proper records are maintained including use of EHR systems for care documentation, medication management and quality initiatives. D. STAFF DEVELOPMENT 1. Provide, plan, schedule, and monitor staff training programs and in-services for the clinical staff including statutory or regulatory required trainings. 2. Attend/complete all required training, in-service, and staff meetings. Training of non-clinical staff may be required as needed. 3. Trains, documents and supervises Care Partners under the Nurse Delegation Act as appropriate. 4. Training and supervision of staff to perform medication assistance services within best practices. EMPLOYMENT REQUIREMENTS: 1. Willingness to adhere to assisted living principles in provided services. 2. Ability to relate to the public, Residents, families, staff, other professionals appropriately. 3. Ability to communicate well, verbally and in writing. 4. CPR/First Aid Certified within 30 days of employment. 5. Ability to lift to 50 pounds occasionally, 30 pounds regularly. 6. Maintains appropriate and required nursing license in state the community is located. TRAINING AND EXPERIENCE: Licensed Nurse with a minimum of at least one-year of leading clinical operations in a senior living setting. Rehabilitative and restorative experience helpful. CPR/First Aid certification. EDUCATION AND LICENSURE: Graduate of accredited nursing school. Maintains current RN/LPN/LVN license in good standing in the state the community is located. Immediately reports to Executive Director any disruption to nursing license status. Provide supervisor of all states you hold an active nursing license/compact license.
