True Hearts and Hands Hospice is excited to announce that we are recruiting for a Registered Nurse/Case Managers with experience serving hospice patients! Positions are PRN to Full-Time, and offer competitive salaries, mileage, and benefits for Full-Time positions.

Registered Nurses:

• Evaluate referrals made to Hospice.
• Under direction of Medical Director, admit patients eligible for hospice services utilizing computer-based EHR system.
• Assess and evaluate patient needs and identify mutually agreed upon goals with patients.
• Develop patient care plans and updates care plans on an ongoing basis;
• Provide superior patient care, and support to patients family
• Utilizing all tools available, effectively communicates patient/family needs and updates to Interdisciplinary Team.
• Participate as a integral part of the True Hearts and Hands Hospice Team.

Position Requirements Include:

• Current Texas State license as a Registered Nurse.
• One year experience as a hospice nurse required, 3 years preferred
• ICU - ER Experience strongly preferred.
• Ability to work in a field setting and proven ability to make sound nursing judgments
• Ability to assess patient needs and formulate individualized patient care plans to meet those needs.
• Effective communication skills
• Familiarity with electronic admission and charting, and basic computer skills
• Current TB Results or chest X-Ray
