JOB SUMMARY:

The Certified Application Counselor acts as a community liaison for Low to Moderate-income participants, connecting them with viable services offered by True Health and throughout the community.

PRIMARY FUNCTIONS:

1. Facilitate patient access to social programs, including Medicaid, Medicare, Social Security, Disability, Food Stamps, Florida KidCare, and Cash Assistance while protecting the fiscal interest of True Health. 
2. Navigate patients through various social program options through one-on-one counseling. 
3. Assess the patient's needs and make referrals to appropriate community resources. 
4. Track and report the patient's progress in applying for assistance. 
5. Advocate on the patient's behalf to help resolve problems with resources. 
6. Attend basic training. 
7. Other responsibilities as assigned 

EDUCATION AND EXPERIENCE:

1. High School Diploma or GED certificate 
2. Some college preferred 
3. Preferred - one or more years of experience as front office staff in a healthcare setting and customer service.

KNOWLEDGE AND SKILLS:

 3. Microsoft Word/Excel or equivalent
