Your Impactful Role as an RN Case Manager:
The RN Case Manager is an integral part of the healthcare team at our Duluth office. Your responsibilities will include:
· Managing and Scheduling Visits: Taking charge of managing and scheduling your patient visits, you'll travel throughout the area to provide personalized 1:1 nursing services in the comfort of their homes. This hands-on approach ensures that our valued patients receive the highest level of care and attention.
· Interdisciplinary Collaboration: Your contribution to the interdisciplinary team is invaluable. By actively collaborating and utilizing your servant leadership and empathetic skills, you foster a supportive and cohesive environment, prioritizing the well-being of our patients and delivering the best possible care.
· Clinical Supervision and Inspiration: Your role involves providing clinical supervision to our caregivers, ensuring they follow the care plan diligently.
