Trove Home Care is Hiring a RN Care Manager ($50-$55/hr) Near Charlottesville, VA

RN Care Manager

Trove Home Care is growing in Charlottesville and looking for an experienced RN to help lead our exceptional team of caregivers. Trove Home Care offers premium non-medical care services for clients in a home-based setting by attracting Central Virginia’s most compassionate caregivers with industry-leading wages and best-in-class training opportunities. The role is primarily remote, outside of new client intakes, and can be flexibly configured between 5 to 15 hours per week around the RN's availability.

Job Duties

  • Conduct new client intakes/assessments.
  • Create detailed Plans of Care for new clients.
  • Monitor daily care logs completed by caregivers to anticipate and/or resolve issues.
  • Work with the scheduler(s) to create optimal care teams based on each client’s needs.
  • Communicate with caregivers prior to their first shift with a new client to clarify any questions about the care plan.
  • Support new caregiver orientations.
  • Conduct occasional supervisory visits with clients and facilitate performance reviews for caregivers.
  • Participate in weekly team meetings.

Pay Range:

  • Commensurate with experience ($50-$55 per hour)

Professional Qualifications (Knowledge, Skills, Abilities)

  • Registered Nurse with a valid license in the State of Virginia
  • Proficient with technology
  • Excellent communication, bedside manner, customer contact skills, and organizational skills
  • Ability to work under pressure to support clients and business needs
  • Experience leading a team and serving as primary point of contact
