Geriatric Psych. Social Worker Needed!

The Geri-Psych Social Worker is a key member of the Care Coordination/Geri-Psych team. The GP SW provides and/or oversees the provision of appropriate and timely resources, interventions and services for selected patients in the hospital. They may also provide therapy or counseling services to patients, as well as help family members learn to deal with mental illness in the family.

The GP SW collaborates with the team of healthcare professionals involved in the plan of care for Geri-Psych patients including, but not limited to, case management and clinical staff, the patient's primary provider, caregivers/family members, practice team members and community partners. In this way, the GP SW ensures that plans are focused and effective which helps patients to connect with and utilize a full range of psychiatric, medical, community and other services available.

Requirements:

• Experience and comfort working with adult patients 55 who have complex psychiatric problems
• MSW required
• LICSW required
• Two years’ experience in Geriatric Psych required
• Knowledge of end of life issues
• Experience in conducting group therapy sessions, family and individual therapy sessions
• Knowledge of community agencies and resources
