Job Description

DIETITIAN

Position:
• Per-Diem/On-call

Job Summary:
As Dietitian you will assist the patient in achieving the best nutritional intake possible given
the current state of his or her health. Dietitians have much experience in dealing with the
obstacles to eating that often challenge the terminally ill. In conjunction with the physician,
the dietitian may make suggestions about food types that are more appetizing to the patient
or ways of preparing or serving the food that make it possible for the patient to eat.

Essential Functions:
• Plan menus to meet the patient’s symptomatic and nutritional needs which support
the palliative treatment of patients.
• Develops and implements a nutritional plan based on an assessment of the
nutritional needs of the individual patients. This plan is included in the patient’s plan
of care.
• Evaluates and reports the results of the nutritional assessment as appropriate.
• Instructs the interdisciplinary team to provide appropriate counseling in nutrition to
families and caregivers in the home setting or facility
• Collaborate with facility dietitian
• Provides in-service education to program staff and educational tools for patients/
caregivers.
• Evaluates and instructs on nutritional principles, dietary plans, food selection, and
economics adapts plans to the individual’s lifestyle and reevaluates nutritional care
by providing home follow-up care.
• Completes appropriate dietary history and nutritional care data and other
documentation as required in the clinical records.
• Attends team meetings and maintains effective communication with other team
members as necessary
• Teaches patients, families and staff special dietary regiments and nutritional
requirements as appropriate to diagnosis and individual preference
Qualifications:
• Current state licensure; Baccalaureate degree from a program approved by the
American Dietetic Association
• Minimum of one year of experience in a health care setting.
• Experience in working with clients who have dietary problems related to illness.
• Previous work with terminally ill patients/families is preferred.
• Knowledge of Enteral /Tube feeds and substitutes.Benefits:
