Trinity Woods is Hiring a Healthcare Move-in Coordinator (Admissions) Near Tulsa, OK

Job Requirements:

  • A degree in a human services field (Nursing or Social Work preferred).
  • Minimum of two (2) years experience in long term care or related programs.
  • Strong communication and public relations skills.
  • Ability to accurately assess and anticipate level of care required for an individual.
  • Must have excellent organizational skills and ability to work independently with little supervision.
  • Ability to work well under pressure.
  • Knowledge of all payor sources including Medicare, Medicaid, HMOs, insurance and private pay.
  • Ability to work in an interdisciplinary setting including marketing, social services, nursing and business affairs.
  • Proficient computer skills including Microsoft Word, Excel, Outlook, etc.

Responsibilities:

 Sales and Marketing

Performs a variety of duties to create a positive “first impression” for Trinity Woods and promote TW’s community recognition on a variety of levels.

  • Develops, coordinates, and implements marketing strategies for referral sources.
  • Maintains data base for referral resources to include inquiries, move-ins and move-outs.
  • Develops and coordinates tours of the Health Center.
  • Maintains positive relationships with area hospital personnel, especially Discharge Planners.
  • Coordinates the move-in process to create a positive experience for new members.
  • Reviews all referral information and coordinates an on-site assessment at the referring facility if indicated.
  • Confer with the CFO to assess each referral for financial feasibility prior to acceptance.
  • Coordinates with Director of Nursing to make all room assignments.
  • Ensures all documentation including Advance Directives, POA/Guardianship, etc. have been discussed and appropriate paperwork is in medical record.

Insurance Verification Responsibilities:

  • Completes insurance authorizations for skilled stays. Manages the review process for each resident stay.
  • Collects required documentation requested by individual insurance groups.
  • Informs nursing and therapy teams of coverage dates.

Additional Responsibilities

  • Meets all resident and household needs by actively assisting (to the extent of cross training and certification) in all tasks of the household.
  • Ensures that resident rights and responsibilities are honored and met.
  • Monitors the community and household response to resident concerns through continuous quality improvement.
  • Maintains professional presence when representing Trinity Woods; exhibits degree of professionalism consistent with standards in performance, behavior, and appearance.
  • Demonstrates professional development/skills competency in all activities.
  • Complies with Trinity Woods’ established safety rules and practices.
