This Chaplain will support our patients primarily in the West Dallas, Irving, Fort Worth, and Arlington areas. This is a newly-created role due to growth and to increase our team member's capacity. HOSPICE EXPERIENCE AS A CHAPLAIN REQUIRED.

Job responsibilities can include but are not limited to:

• Asses patient/family to identify spiritual needs and develops a spiritual plan of care.
• Collaborates with local clergy as needed.
• Conducts services of prayer, worship and ritual for patient/families, as appropriate.
• Provides individual patient/family counseling, crisis intervention, information and assistance with preparation of advanced directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Work with IDT, participates in the ongoing POC by developing, documenting and evaluating responses to interventions in order to meet spiritual needs of the patient and family.
• Provides consultation, education, and support to the interdisciplinary team on spiritual care and bereavement counseling.
• Makes visits to patient/family prior to death and will be available to family during the bereavement period as needed.
• Comply with applicable legal requirements, company standards, policies and procedures

Qualifications:

• Qualified candidates will have at least one year of experience as a Chaplain in a hospice setting
• Masters of Divinity preferred
• Must have a current Virginia Driver's license and dependable transportation
