Description:

Mercy Home Health, a member of Trinity Health at Home, has an immediate need for a Full-time License Practical Nurse (LPN) for the greater Philadelphia area. This position is based out of our Broad Street branch office.

The LPN preforms varied nursing care activities and related support services for patients in their home. The LPN collaborates with and works under the direct supervision of a Registered Nurse.

REQUIREMENTS

  • High School diploma or equivalent with current LPN Licensure in the Commonwealth of Pennsylvania with Current BCLS certification/CPR required.
  • Valid driver's license and car insurance also required.
  • Experience in Home care preferred.

Provides primary nursing care to home based care patients as ordered by the physician, and in accordance with the organization’s policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. Provides functional support/supervision to Home Healthcare Aides and LPNs as needed. Provides therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.
