Trinity Health Michigan is Hiring a RN (Registered Nurse) Case Manager - Part Time Near Grand Haven, MI
Shift: Days
What you will need:
- RN with Michigan license, in good standing, Bachelor’s degree in Nursing preferred
- Four years recent relevant clinical experience required; utilization review experience, discharge planning, or case management experience preferred.
- Possesses effective interpersonal skills, can work across departmental boundaries, facilitates problem resolution, and maintain a professional demeanor in difficult situations.
- Knowledge of the functional operations of utilization management, discharge planning, care coordination and transitions of care preferred.
What you will do:
- Performs assessment of the medical record to determine that the documentation supports the severity of illness and intensity of service criteria for admission per payer contracts/integral criteria
- Complete utilization review and discharge planning documentation as needed. Performs subsequent review of the medical record for continued stay and discharge plan per payer contracts and InterQual criteria
- Liaison between payers and providers for payer requirement and standards.
