Trinity is currently hiring a full-time SNUG Hospital Responder, within our SNUG Outreach Team. The Hospital Responder functions as an outreach worker with a focus on working with victims of violence in the hospital. The primary role of the hospital responder is to prevent retaliation and shootings and to assist victims and family members of those who have been injured or killed. Although the Hospital Responder will have similar duties as a SNUG Outreach Worker (canvass with the team, attend briefings, participate in all trainings), community outreach work will be limited, when necessary, to prioritize hospital responses. The Hospital Responder's caseload will focus on work with patients discharged from the hospital.

What will you be doing as a Hospital Responder?

• Work closely with the hospital-based SNUG Social Worker and other hospital staff
• Prioritize hospital responses when victims of violence present at the hospital
• When appropriate, connect with SNUG team about mediations that should be done in the community
• When necessary, assist in verbally de-escalating family and friends after an incident; frequently this will be in the emergency room or hospital parking lot
• Visit patients and families daily over the course of the hospital stay
• Provide patients with general support including information about victim services, trauma responses, advocacy within the hospital, and connections to community resources
• Abide by all hospital rules including visiting requirements, discharge planning, and safety precautions
• When indicated, ensure there is a connection to the SNUG outreach team prior to the patient leaving the hospital
• Follow-up with patients and families post-discharge and assist with scheduling and transportation to follow-up appointments

What are your skills and experience?

• Experience working with at-risk youth and gang members
• No pending criminal cases or prior convictions for sexual assault, child abuse or domestic violence
• Excellent communication skills
• Experience or training in crisis intervention
• Adherence to substance abuse policy.
• Knowledge of target communities
• Valid New York State driver's license, insurance, and good driving record

Salary - $50,000/year
