The Community Outreach Specialist is responsible for all aspects of Signature marketing and sales including establishing, maintaining and cultivating positive referral source relationships on behalf of the location to achieve the locations budgeted admissions and census, while also responding to customer requests and concerns.

Responsibilities:

Develops a defined territory of referral sources targeting hospitals, skilled nursing facilities,senior communities, medical clinics, and physicians to generate admissions to meet or exceed targeted monthly goalsGenerates marketing and promotional initiatives to achieve budgetary volume projectionsContinuously conducts market assessments and assists in developing a comprehensivemarketing plan designed to meet budgetary volume projections and identifying opportunities foradditional or improved services to address customer needsFacilitates communications and works effectively between employees / volunteers, patients /families / caregivers, physicians, vendors, referral sources and other departments andprofessionals as appropriateConsistently represents Signature with integrity and professionalism to all internal and externaland current and potential customersAssists the Direct Supervisor in establishing program volume projections in the annual budgetand in establishing allocations for the marketing department. Monitors allocation of resourcesaccording to budgetary limitationsAssists with and participates in provision of community, professional and educational outreachactivities.Builds and monitors community and market perceptions of Signature as a high-quality providerof servicesMaintains comprehensive working knowledge in the field of marketing and of Signature policiesand markets including government agencies and regulations, major payer groups, key referralsources, and competitors market positioning. Serves as a resource for customers and thecommunity and with appropriate organization personnelGenerates appropriate referrals to Signature from current and new customers within territoryParticipates in the recruitment, selection and orientation of organization personnelActively participates in staff meetings, department meetings, in-services, QAPI activities, SafetyCommittee and other related activities as applicableMaintains comprehensive working knowledge of community resources and assists customers inaccessing community resources should services not be provided by SignatureParticipates in evening/weekend on-call for admissions as requiredAccepts and performs other related duties and responsibilities as assigned by the DirectSupervisorAccepts direct responsibility for the growth and overall success of SignatureResponsible for growing a qualified referral stream to the agency bya) growing referrals within existing accountsb) growing new accountsProvides reports on time in an organized and correct manner to direct supervisor as requestedSeeks partnerships with hospitals, SNFs and other referral sources, ACO, insurance companiesand moreSupports mission, values and goals of Signature

Qualifications/Skillsets:
  • EDUCATION and EXPERIENCE :

    • Prefer Bachelor's degree in Marketing, Business Administration, or related field.
    • At least three (3) to five (5) years of experience in health care marketing management, preferably in hospice, home health & home care operations.

    QUALIFICATIONS and SKILLS:

  • Ability to manage payer mix of patients
  • Ability to market aggressively and deal tactfully with customers and the community
  • Knowledge of corporate business management
  • Demonstrates good communications skills, negotiation skills, and public relations skills
  • Capable of performing the Essential Job Functions / Responsibilities of this job, with or without reasonable accommodation
  • Demonstrates autonomy, organization, efficient time & territory management, assertiveness, flexibility and cooperation in performing job responsibilities
  • Possess basic computer skills and knowledge of office technology e.g. email and internet services, facsimile machines, telecommunication devices,
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements
