Job Summary and Qualifications

The Electrophysiology Nurse Navigator will be the liaison between the patient and the Cardiologist and ensure the patient and his or her family receives exceptional patient care.

What you will do in this role:

• You will serve as the primary contact person for the electrophysiology/cardiology team, including physicians, patients, and other health care providers to help ensure good coordination of inter-departmental processes and communication.
• You will provide oversight of HF/arrhythmia clinic schedule and triage.
• You will serve as a clinical leader with representation and promotion of education in the hospital as well as the outpatient environment as it relates to cardiac dysrhythmias.
• You will work closely with ancillary departments and services to assure optimal continuity of care of patient in the hospital setting including in-patient, procedural, post hospital discharge and clinic settings.
• You will plan, coordinate, and implement care of cardiac dysrhythmia patients in collaboration with physician team from the outpatient clinic setting.
• You will participate in evaluation and outcome monitoring of the provided clinical care including quality initiatives and help manage clinical databases.

What qualifications you will need:

• Must be a graduate from an accredited school of Nursing: Associate's Degree in Nursing or higher is required
• Must be licensed as a Registered Nurse in the state which you practice and/or hold an active Compact/Multi-State RN license
• A current American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) Health Care Provider is required
• You must have previous nursing experience in cardiovascular and EP related patient population
