Title : LVN/LPN – Behavioral Health

Duration : Full Time Permanent

Shift : 12 Hrs. (3 Days/Nights per week)

Salary Range: Min Pay: $22.66/hour (with 0 years of experience) - Max Pay: $34.04/hour (with 24 years of experience).

Job Description:

· Primary function is to administer skilled nursing care to patients in their place of residence, as prescribed by the physician and in accordance with the established plan of care.

· The LPN/LVN works under the supervision of a Registered Nurse (RN) and coordinates care with the interdisciplinary team as well as the patient/family.

· The LPN/LVN will deliver care in compliance with State, Federal and Medicare standards.

· Under the direction of the RN, assists in providing nursing services and identifying the patient's physical, psycho-social and environmental needs as evidenced by observation, documentation, clinical record, case reference, team report and evaluation.

· Reports outcomes to assigned RN Case Manager.

· Participates in planning, coordinating, and implementing care in conjunction with the RN, to provide quality, cost-effective patient care.

Requirements:

· Some Psych/Behavioral Health Experience – Required.

· Experience working with child and adolescent populations - Required

· CPI - please confirm they are willing to obtain onsite if they don't already have this certification. – Required.

· COVID vaccination and Booster – Required.
